阿托品防治青少年近视疗效的临床分析

摘　　要：目的探讨阿托品治疗青少年近视的有效性、安全性及作用机制。方法选择2009年1月至2010年2月在我科门诊就诊确诊为近视眼的8～18岁患者。等效球镜度数≤6.00 D,散光≤1.00 D患者162例(324眼)。随机分为治疗组、对照组各81例(162眼)。治疗组予10 g/L硫酸阿托品眼用凝胶点眼,每日1次,3个月为1个阶段,停用1个月后,再次应用,反复应用4个阶段后结束治疗。检测、记录用药前后视力、屈光度、眼轴、眼压等。结果结束治疗后,治疗组在治愈、进步、下降率等各项指标中明显优于对照组,两组视力下降率分别14.81%、56.62%;对照组屈光度平均增加-1.3 D,眼轴长平均增加0.81 mm;组间进行数据比较差异有统计学意义(P<0.05)。眼压正常,晶状体、视网膜无损害。结论阿托品长期、规律性眼部应用,能有效地控制并减缓青少年近视发展,无不良反应,价格低廉,使用方便,值得推广。机制与减轻调节、延缓眼轴增长速度有关。Objectlve To explore the effectiveness of atropine for treatment of juvenile myopia, safety and mechanism. Methods From January 2009 to February 2010 in Department of Ophthalmalogy, Qinzhuu City Second People Hospital patients of 8 to 18 years diagnosed as nearsighted were chosen. 162 cases（324 eyes）of spherical equivalent degree ≤6.00 D,astigmatism ≤1.00 D were randomly divided into treatment groups and control group,each of 81 cases（ 162 eyes）. Treatment group was given 10 g/L of atropine sulfate eye gel, once a day,3 months as 1 stage, then the administration stopped for 1 month, then the application cy- cle was repeated, altogether for 4 stages. Visual acuity, refraction, axial length, intraocular pressure were test- ed and recorded before and after the treatment. Results At the end of treatment, the cure, progress, decline rate and other indicators of the control group were all obviously better than the control group, vision loss of the two groups were 14.81% ,56.62% respectively;diopter of the control group increase - 1.3 D on average, ax- ial length increased 0.81 mm on average;the differences between the groups were statistically significant （ P 〈 0.05 ）. The intraocular pressure was normal, no lens or retinal damage occurred. Conclusion Long and regular application of atropine on eye can effectively control and reduce the development of myopia, without adverse reactions, and with the advantages of low cost and easy application, which is worth promoting. The mechanism is related to the reduction, regulation, and slowing down the speed of axial growth.
