核块内包埋法在硬核白内障超声乳化术中的应用

摘　　要：目的探讨核块内包埋法在硬核白内障超声乳化手术中的应用价值。方法选择硬核老年性白内障患者180例(194眼),随机分为核块内包埋法超声乳化组和常规超声乳化组。观察术中后囊膜破裂发生率,术后角膜水肿发生率,并对结果进行分析。结果核块内包埋法超声乳化组后囊膜破裂2例(2.06%),常规超声乳化组后囊膜破裂8例(8.25%),术后第1天,核块内包埋法超声乳化组角膜轻度水肿23眼(23.71%),无明显水肿和角膜失代偿;常规超声乳化组角膜轻度水肿者48眼(49.48%),明显水肿者35眼(36.08%),发生角膜失代偿2眼(2.06%)。两组间比较有统计学差异(P<0.05)。结论核块内包埋法超声乳化可减少术中后囊膜破裂发生率,显著降低术中产生的热量对角膜内皮层的热损伤,提高术后视觉质量,值得在临床推广使用。Objective To investigate the effect of embedding in nucleus method for phacoemulisification to hard nucleus cataract. Methods The 180 patients（194 eyes） of cataracts with nucleus density at grade Ill-IV were randomly divided into the nucleus embedding group and the routine method group. The rupture of posterior capsule and cornea edema after operation were compared between both groups. Results The corneal edema in the embedding in nucleus method group after operation was significantly lighter than that in the routine method group; there was sisnificant difference in the eomplication＇s incidence between both groups （P〈0.05）. Conclusion The embedding in nucleus method for phacoemulisification can reduce the rupture of posterior capsule and heat damage on corneal endothelium. It can improve the vision significantly and is worthy to be applied widely in clinic.
