摘 要:目的:探讨前列腺癌表观扩散系数(ADC)值与前列腺特异性抗原(PSA)浓度的相关性。方法:回顾性分析经穿刺活检、手术病理证实的36例前列腺癌患者的临床及影像资料,记录前列腺癌兴趣区的ADC值,测量患者PSA浓度,统计分析各期(TNM分期)前列腺癌病灶ADC值与PSA的相关性。结果:T2期22例,平均ADC值为(0.70±0.08)×10-3 mm2/s,平均PSA值为(70.0±10.7)ng/ml;T3期6例,平均ADC值为(0.76±0.09)×10-3 mm2/s,平均PSA值为(301.0±20.1)ng/ml;T4期8例,平均ADC值为(0.72±0.08)×10-3 mm2/s,平均PSA值为(360.0±25.1)ng/ml。ADC值与PSA浓度在前列腺各期中均存在负相关关系,r值分别为-0.638、-0.674和-0.731,P值均<0.01。结论:前列腺癌病灶的ADC值与PSA浓度呈负相关,根据ADC值可对前列腺癌的生物学特性进行初步评估,可为临床制定合适的治疗方案提供相关信息。Objective:To study the correlation between apparent diffusion coefficient (ADC) value and prostate spe- cific antigen (PSA) level in prostatic cancer. Methods:The clinical and imaging data of 36 patients with biopsy or surgical pathology proven prostatic cancer were studied retrospectively. The ADC in the region of interest of prostatic cancer were recorded and PSA level was measured. The correlation of ADC and PSA level of each stage (TNM staging) in prostatic cancer were statistically analyzed. Results:In 22 cases with stage T2,the mean ADC was (0.70±0.08)×10^-3mm^2/s,the mean PSA level was (70.0±10.7)ng/ml. In 6 cases with stage T3,the mean ADC was (0.76±0.09)×10^-3mm^2 /s, the mean PSA level was (301.0±20.1)ng/ml. In 8 eases with stage T4,the mean ADC was (0.72±0.08) ×10^-3mm^2/s,the mean PSA level was (360.0±25.1)ng/ml. Negative correlation was found between the ADC and PSA level in each stage of prostatic cancer (r=- 0.638,-0. 674 and -0.731, P〈0.01). Conclusion: There is negative correlation between ADC val- ue and PSA level in each stage of prostatic cancer. It can be used to estimate the biological behavior of prostatic cancer,and is useful for the therapeutic planning.
