摘 要:目的:研究旋转容积调强(RapidArc)在脑胶质瘤中应用的适用性和剂量学特性。方法:对10例脑胶质瘤患者分别设计RapidArc和固定野逆向调强放射治疗(dIMRT)计划。利用剂量体积直方图比较两种计划中靶区、危及器官的剂量学差异,并比较两种治疗计划的总机器跳数(MU)和治疗时间。结果:与dIMRT相比,RapidArc的CTV1平均剂量Dmean,CTV2平均剂量Dmean、最小剂量Dmin稍高,差异有统计学意义(P=0.017,0.036,0.002);CTV2的适合度指数CI值更接近于1(P=0.007);RapidArc中危及器官各指标之间仅有患侧视神经Dmax稍高(P=0.003);其余各指标间差异无统计学意义(P>0.05)。机器跳数RapidArc平均减少了40.9%,治疗时间约是dIMRT的1/3。结论:在脑胶质瘤中,RapidArc计划可以达到或优于dIMRT计划的剂量分布,满足临床剂量学要求,并且具有较少总MU、总治疗时间的优势。Objective:To study the application suitability and dosiology characteristics of RapidArc in human gliomas. Methods : We designed separately RapidArc plan and fixed - gantry dynamic intensity modulated radiation therapy(dIMRT) plan for 10 glioma patients. The posology differences of the two plans in target areas and organ threatening were studied by dose -volume histograms, and the total numbers of hops machine (MU) and treatment times of the two plans were compared. Results:Compared with dlMRT, the Dmean of CTV1, Dmean and Dmin of CTV2 of RapdiArc were all higher, and the difference was statistically significant ( P = 0. 017,0.036,0. 002) ; The CTV2 Is CI value of RapidArc was closer to 1 ( P = 0. 007) ; Only Dmean of ipsilateral opticnerve of RapidArc was slightly higher among all the indexes of organ threatening( P = 0.003 ) ; There was no significantly statistical difference for all the other indexes (P 〉 0.05 ). The machines hops'number of RapidArc reduced averagely 40.9%, and the treatment time of RapidArc was about 1/3 of dIMRT's treatment time. Conclusion:In human gliomas, RapidArc plans can meet or exceed the dose distribution of dIMRT plan, satisfy the requirements of clinical dosimetry, and has the advantages of less total MU and less total treatment time.
