替吉奥联合奥沙利铂及多西紫杉醇治疗晚期胃癌的临床观察

摘　　要：目的:观察替吉奥联合奥沙利铂及多西紫杉醇治疗晚期胃癌的近期疗效和安全性。方法:对24例晚期胃癌患者进行随机对照研究。试验组14例:替吉奥胶囊80mg/(m2.d),分2次服用,d1-14;奥沙利铂130 mg/m2,静脉滴注,d1;多西紫杉醇75mg/m2,静脉滴注,d1。对照组10例:5-FU 2400 mg/m2,静脉滴注,d1-5,奥沙利铂及多西紫杉醇用法同试验组。两组均3周为一个周期。治疗2个周期后评价疗效和毒副反应。结果:试验组和对照组的总有效率分别为50%和30%,差异无统计学意义(P>0.05)。试验组的骨髓抑制和口腔黏膜炎发生率分别为71.4%、14.3%,显著低于对照组(90%、80%,均P<0.05),而胃肠道反应、脱发、肝功能损害等方面差异不明显(P>0.05)。结论:替吉奥胶囊联合奥沙利铂及多西紫杉醇治疗晚期胃癌是安全、有效的,远期疗效有待进一步观察。Objective：To evaluate the efficacy and safety of S - 1 combined with oxaliplatin and docetaxel in the treatment of patients with advanced gastric cancer. Methods： All 24 cases with advanced gastric cancer diagnosed and treated in the Dezhou Peopleg Hospital between October 2009 and January 2011 were enrolled in this randomized controlled trial. The trial group（14cases） ：S- 1 capsule 80 rag/（ m^2 · d）, twice daily , d1 - 14 ;oxaliplatin 130 mg/m^2, intravenous drip , d1 ; docetaxel 75 mg/m^2, intravenous drip, dI. The control group （ 10 cases） ： 5 - FU 2400 mg/m^2, intravenous drip, d1-5 ; oxaliplatin 130 mg/m^2, intravenous drip, d2 ; docetaxe175 mg/m^2, intravenous drip, d1· One treatmerit cycle covered 3 weeks. The effects and toxicities were evaluated after 2 circles. Results： The effective rates of trial group and control group were 50% and 30% , respectively（ P 〉 0.05 ）. The incidence rate of adverse effects such as bone marrow suppression and oral mucositis in the trial group was significantly lower than that in the control group（ P 〈 0.05 ） , while there was no significant difference regarding the gastrointestinal toxicity, hair loss, and liver damage （ P 〉 0.05 ）. Conclusion： S - 1 capsule combined with oxaliplatin and docetaxel is safe and effective in the treatment of advanced gastric cancer. However, the long - term efficacy of this regimen should be further observed.
