热放化疗治疗中晚期宫颈癌近期疗效

摘　　要：目的:探讨盆腹腔深部热疗联合同步放化疗治疗中晚期宫颈癌的疗效和安全性。方法:对2007年2月至2010年10月102例中晚期宫颈癌患者随机分成热放化疗组50例(治疗组)及放化疗组52例(对照组),两组在同样放化疗基础上,治疗组于化疗当天行盆腹腔深部热疗,每次60min,1次/周,共6次。结果:治疗结束后治疗组CR 32例,PR 10例,总有效率84.0%;对照组CR 20例,PR 12例,总有效率61.5%;两组差异有统计学意义(P<0.05)。治疗结束后1个月,治疗组CR 39例,PR 8例,总有效率94.0%;对照组CR 28例,PR 10例,总有效率73.1%;两组差异有统计学意义(P<0.05)。治疗结束3个月时,两组的有效率分别是96.0%、78.8%,两组差异有统计学意义(P<0.01)。两组均有骨髓抑制和消化道反应,经治疗后均可耐受。结论:盆腹腔深部热疗联合同步放化疗可显著提高中晚期宫颈癌患者的近期疗效,早期毒副反应并无增加。Objective：To study the curative effect and the security of deep hyperthermia in pelvic cavity and abdominal cavity combined with radiotherapy and chemotherapy for treating the mid and late stage cervical cancer patients. Methods：All 102 cases of the mid and late stage cervical cancer patients in our hospital from February 2007 to October 2010 were divided into the hyperthermia combined with radiotherapy and chemotherapy group（50 cases ,treatment group） and radiotherapy combined with chemotherapy group （52 cases, control group） stochastically, the similarly radiotherapy and chemotherapy for two groups,performed deep hyperthermia in pelvic cavity and abdominal cavity for the treatment group at the same day of chemotherapy ,60min each time, once/week ,6 times altogether. Results：After the treatment,there were 32 cases of CR and 10 cases of PR in the treatment group, total effective rate was 84.0% ,20 cases of CR,12 cases of PR in the control group,total effective rate was 61.5%, （P 〈0.05）. After the treatment had finished 1 month, there were 39 cases of CR and 8 cases of PR in the treatment group, total effective rate was 94.0%, 28 cases of CR andl0 cases of PR in the control group ,total effective rate was 73.1%, （P 〈0.05）. When the treatment had finished for 3 months,the total effective rate was 96.0% and 78.8%, （P 〈0.01 ）. In two groups patients had marrow suppression and digestive tract response, and be controed after treatment. Conclusion：For mid and late stage cervical cancer patient＇s short - term curative effect can be improved obviously by hyperthermia, radiotherapy and chemotherapy treating synchronously, and the adverse reactions will not increase.
