42例肺癌脑转移的放射治疗疗效分析

摘　　要：目的:评价肺癌脑转移放射治疗患者的生存率和肿瘤局控率。方法:回顾性分析42例接受全脑放射治疗的同时行立体定向放射治疗的肺癌脑转移瘤患者临床资料。结果:42例患者的中位生存期为17个月(95%可信区间11.27～20.73)。全组患者的肿瘤局部控制中位时间为21个月(95%CI:18.21～22.45)。6个月、1年和2年的肿瘤局部控制率分别为96.6%、82.5%和48.9%。治疗后1个月后,健康相关生活质量评分较治疗前明显改善,差异有统计学意义(P<0.05)。结论:全脑放射治疗同步立体定向放射治疗肺癌脑转移瘤,能控制脑转移病变的生长,提高患者的生活质量。有选择地进行全脑放射治疗同步立体定向放射治疗脑转移瘤是安全有效的。Objective： Evaluation of lung cancer with brain metastasis radiotherapy survival and tumor local control rate.Methods： The review analysis 42 examples accept the entire brain radiotherapy the same fashionable three-dimensional direction detection radiotherapy brain to shift the lump.Results： 42 patient＇s position lifetime was 17 months （95% confidence interval： 11.27-20.73）.The median time of local tumor control was 21 months（95% confidence interval： 18.21- 22.45）.The six-month, one-year and two-year local control rates were 96.6%, 82.5% and 48.9%.Heahh related quality of life（HRQOL） was improved one month after radiotherapy（P〈0.05）.Conclusion： The all the brain radiotherapy a three-dlmensional redirection radiotherapy for lung cancer with brain tumour transfer can control the brain lesions the transfer, improve the quality of life.Selectively on the brain radiotherapy a three-dimensional redirection radiotherapy brain tumour is safe transfer of effective.
