脊柱神经管闭合不全的MRI诊断

摘  要:目的 :研究脊柱神经管闭合不全的MRI诊断。方法 :使用PhilipsGyroscanACS NT  15型1.5T超导系统、Synergy专用脊柱线圈对 76例脊柱神经管闭合不全患者进行MR检查。 结果 :脊髓空洞症39例 ( 1例合并脊髓纵裂 ) ,脊髓纵裂 6例 ,脊髓栓系综合征 30例 (其中 2 8例合并其他SD病变 ) ,椎管内胚层组织肿瘤 30例 ,其中脂肪瘤 14例、表皮样囊肿 7例、皮样囊肿 2例、肠源性囊肿 3例、畸胎瘤 4例。皮样窦道 2例 ,骶尾骨发育不良 4例 ,脊膜膨出 4例 ,脊髓脊膜膨出 2例 ,脂肪脊髓脊膜膨出 8例 ,对其MR表现进行分析。结论 :MRI能正确、全面地诊断包括椎体畸形在内的大多数脊柱神经管闭合不全病变 。Objective:To investigate highfield MR imaging characteritics of 76 patients with spinal dysraphism(SD).Methods:1.5T Philips Gyroscan ACSNT 15 MRI unit was utilized.Results:There were 39 cases of syringomyelia (one patient with complicated diastematomyelia),6 diastematomyelia and 30 cases of tethered cord syndrome(28 patients with other SD lesions ).Lipomas(14 cases)accounted for 46.7% of all the intraspinal germ cell tumors (30 cases ).Seven cases of epidermoids and two dermoids,three enterogenic cysts,four teratomas were also reported.Conclusion:All the cases with menigocele,myelomenigocele and lipomyelomenigocele could be well defined by MRI.Other deformities including sacral dysgenesis(4 cases )and dermal sinus(2 cases)were also delineated.Most lesions of SD including bony deformities could be diagnosed with MRI.But the differential diagnosis of epidermoid between dermoid was still difficult.
