甲状腺微小癌术中冰冻取材及诊断体会

摘　　要：目的探讨冰冻切片在甲状腺手术中的应用及甲状腺冰冻取材的一些技巧及注意事项,以此提高甲状腺微小癌术中冰冻诊断的准确率,减少恶性肿瘤的漏诊率。方法回顾性对比观察病理科近2年87例甲状腺微小癌患者肿物冰冻切片和石蜡切片,并回顾其冰冻取材记录。结果冰冻切片诊断甲状腺微小癌82例,延迟2例,漏诊2例,假阴性诊断1例,冰冻切片确诊率为94.3%。结论正确规范的取材方法及冰冻制片方法可提高对甲状腺微小癌的判别能力及甲状微小癌术中诊断的准确率,减少甲状腺微小癌的术中漏检率,适时指导临床手术。Objective To discuss the application of frozen section in thyroid operations, the skills and attentions of tissue preparation, so as to increase the accuracy rate of the diagnosis of thyroid microcarcinoma and reduce the missed diagnosis rate of cancer. Methods The histopathological diagnosis results of 87 cases of thyroid microcareinoma by frozen sections and paraffin sections were analyzed retrospectively. The records of tissue preparation were also reviewed. Results There are 82 eases of thy- roid microcarcinoma were diagnosed by frozen section, while 2 cases were delayed ,2 cases were missed, and 1 case was false neg- ative diagnosed. The positive rate for frozen section was 94.3%. Conclusion The frozen section with standard preparation of tis-sue can improve the discrimination and accurate rate of thyroid microcareinoma diagnosis in operations, decrease the missed diag-nosis rate and guide the clinical surgery timely.
