摘  要:目的高速公路上交通事故所致的高速打击眼钝挫伤临床预后较差。文中通过对高速枪弹伤眼球钝挫伤动物模型中眼视网膜色素细胞超微结构改变的对比观察,探讨眼球钝挫伤损伤机制。方法用汽步枪以90 m/s的速度射发TB弹(平均重量0.20122g)击中新西兰实验兔角膜中央区,制作高速枪弹伤眼球钝挫伤动物模型。随机分为实验组8只,对照组2只,单眼制伤或处理。分别于伤后4个不同时间点处死实验组动物2只,取后极部全层球壁组织,2%戊二醛固定,透射电镜下对比观察视网膜色素细胞超微结构改变。结果高速枪弹伤眼球钝挫伤后实验动物眼视网膜色素细胞出现了一系列超微结构改变,主要表现为伤后1 d大量离断的视网膜视细胞末节散布于视网膜色素细胞内;伤后3 d视网膜色素细胞内膜性细胞结构肿胀显著;伤后7 d视网膜色素细胞内膜性细胞结构呈模糊状增厚明显;伤后14 d视网膜色素细胞结构模糊,仍见大量视网膜视细胞末节碎片未降解。结论高速枪弹伤眼球钝挫伤动物模型实验眼视网膜色素细胞超微结构改变显著,可为临床病情的诊断提供参考。Objective The prognosis of eyeball confusion induced by high-speed strike in traffic accidents is poor. The pur- pose of this study was to investigate the mechanism of eyeball contusion by observing the ultrastructural changes of retinal pigment cells in rabbits with eyeball contusion induced by high-speed bullets. Methods We made an animal model of eyeball contusion by hitting the eyes of rabbits with plastic bullets (weighing 0.20 -122 g) from a fixed air rifle at a speed of 90 m/s. Eight rabbits were included in the experimental group, which were treated unilaterally, and 2 in the control group. We sacrificed 2 experimental animals at 1, 3, 7 and 14 d after modeling, respectively, harvested the full posterior pole of the eye wall for 2% glutaraldehyde fixation, and observed the uhrastructural changes of retinal pigment ceils under the transmission electron microscope. Results A series of uhrastructural changes were observed in the retinal pigment cells of the experimental animals after high-speed bullet injury : a large number of disartic- ulated distal photoreceptor cells scattered in the retinal cells at 1 d; the intimal structure of retinal pigment epithelial (RPE) cells markedly swollen at 3 d, thickened in a fuzzy manner at 7 d ; the structure of the retinal pigment ceils indistinct at 14 d, with a large number of undegraded retinal distal fragments of photoreceptor cells. Conclusion Eyeball contusion induced by high-speed bullets may result in significant uhrastructural changes of retinal pigment cells in animal models.
