原发性翼状胬肉760例术后复发情况分析

摘　　要：目的评价原发性翼状胬肉切除后单纯巩膜暴露法、羊膜移植法及带蒂结膜瓣转移术3种手术方式对其复发率的影响。方法随访门诊2009～2011年749例(760眼)原发性翼状胬肉术后的复发情况,比较不同手术方式治疗原发性翼状胬肉的复发率并对结果做统计学分析。结果单纯巩膜暴露法复发率11.43%(24/210);羊膜移植法复发率5.19%(7/135);带蒂结膜瓣转移术复发率3.37%(14/415)。3种不同手术方式术后复发率差异有统计学意义(χ2=16.401,P=0.0003),其中羊膜移植法与带蒂结膜瓣转移术在治疗翼状胬肉中的疗效相近,复发率较低,两组间复发率差异没有统计学意义(χ2=0.910,P=0.34)。结论羊膜移植法与带蒂结膜瓣转移术在治疗翼状胬肉中的疗效相近,较之单纯巩膜暴露法复发率低。Objective To evaluate the effects of three operation methods which include simpled sclera exposure, amniotic membrane transplantation and conjunctival flap transference on the recurrence of primary pterygium after excision. Methods Seven hundred and forty-nine cases （760 eyes） of primary pterygium after surgery was followed up in the outpatient Opthalmology Department of Jiangsu Provincial Hospital from 2009 to 2011. The recurrence with different operation methods were compared. Results The recurrent rate of simple scleral exposure, amniotic membrane transplantation and eonjunctival flap transfer was 11. 43% （24/210）, 5. 19% （7/135） and 3. 37% （14/415） respectively. The Chi square test showed significant differences among these three different operation methods （X2 = 16.401,P = 0.000 3 ）. The a mniotic membrane transplantation and the conjunctival flap transference methods had similar curative effect with low recurrence rate and showed no significant difference in the recurrence rate（x2 = 0. 910,P = 0.34）. Conclusions In treating pterygium, the amniotic membrane transplantation and the conjunctival flap transference methods had similar curative effect and lower recurrence rate than the simple sclera exposure method. （ Chin J Ophthalmol and Otorhinolaryngo1,2013,13：24-26）
