RIA和RT-PCR检测大肠癌患者CEA和CEAmRNA的比较

摘　　要：目的　用放射免疫法 (RIA ,radioimmunoassay)和逆转录多聚酶链反应 (RT -PCR ,reversetranscription -polymerasechainreaction)技术 ,分别检测大肠癌患者外周血中的癌胚抗原 (CEA ,carcino -embryonicantigen)和CEAmRNA ,比较两种技术的敏感性。材料和方法　采用RIA和RT -PCR技术分别检测 44例大肠癌患者外周血的CEA和CEAmRNA ,肿瘤按手术标本浸润深度分壁内型和壁外型。壁内型 :14例 ,壁外型 :30例。结果　 1 用RT -RCR技术可从 1× 10 7个正常血液单核细胞中检出 10 0个肿瘤细胞 ,即可从大约 5ml血液中检出低至 10 0个LoVo细胞产生的CEAmRNA ;2 CEA和CEAmRNA的阳性表达率分别为 31 8% (14 /44 )和 6 5 9% (2 9/44 ) ,二者存在显著性差异 ,RT -PCR与RIA检测CEA的符合率为 78 6 % (14例RIA检测阳性的病例中有 11例RT -PCR阳性 ) ;3 CEAmRNA在壁内型和壁外型的表达率分别为 35 7% (5 /14 )和 80 % (2 4/30 ) ,两型的表达率存在显著性差异 ;而CEA在壁内型和壁外型的表达率分别为 35 7% (5 /14 )和 30 % (9/30 ) ,两型的表达率无显著性差异。结论　 1 RT -PCR技术检测大肠癌患者外周血中的CEAmRNA较RIA检测CEA具有较高的敏感性。2 CEAmRNA的检出率随病程的进展而升高 。Aim RIA(radioimmunoassay)was used to detect the CEA(carcino embryonic antigen)and RT PCR(reverse transcription polymerase chain reaction)to detect CEA mRNA in the circulating tumour cells in patients with colorectal carcinoma.The sensitivity of the two techniques were analysed.Methods CEA and CEA mRNA of tumor cells in peripheral blood of 44 cases patients with colorectal carcinoma were detected by RIA and RT PCR respectively.According to the invasive depth in the wall of large intestine,tumours were divided into two classes:in wall and out wall.14 cases were in wall and 30 cases were out wall.Results ①CEA mRNA could be detected by RT PCR at a concentration of as low as 100 tumour cells in 1×10 7 normal monocytes,which means 100 tumour cells could be detected in 5 ml normal peripheral blood.② The positive rate of CEA and CEA mRNA were 31.8%(14/44)and 65.9%(29/44)respectively.The two rates were of significant difference( P <0.05) The positive rates of detection by RIA and RT PCR for CEA was 78.6% in consistency(11 cases were positive by RT PCR in 14 cases positive by RIA).③ The positive rates of detection for CEA mRNA by RT PCR in the in wall and out wall patients were 35.7%(5/14) and 80%(24/30),respectively.The two rates were of significant difference( P <0.05),but the positive rates of detection for CEA by RIA were 35.7%(5/14)and 30.0%(9/30) respectively,and the two rates were not of significant difference( P >0.05) Conclusion ① RT PCR technique to detect CEA mRNA is more sensitive than the RIA to detect CEA in the peripheral blood in the patients with colorectal carcinoma.② The expression of CEA mRNA was increased with the tumour's progression,but the CEA wasn't.
