非溃疡性消化不良患者体表胃电图与幽门螺杆菌关系的研究

摘　　要：目的　探讨幽门螺杆菌 (Hp)对非溃疡性消化不良 (NUD)患者体表胃电图的影响。方法　通过胃镜活检胃窦粘膜进行快速尿素酶试验及Giemsa检查以判定Hp感染状态 ,并对NUD患者空腹体表胃电图与Hp二者的关系进行了评价。结果　 35例NUD患者Hp阳性 (Hp + ) 2 4例、Hp阴性 (Hp -) 11例 ,Hp +NUD患者AP值明显高于Hp -NUD患者及健康人 (胃体 :2 6 9 6 1± 81.11μv、14 8 10± 6 1.32 μv、15 6 75± 71.73μv ;胃窦 :2 72 94± 6 8.45 μv、15 0 41± 71.11μv、193 5 0± 98.74μv ,P <0 .0 1) ;而FP值前者显著低于后两者 (胃体 :1 77± 0 .36cpm、2 80± 0 .77cpm、2 93± 0 .38cpm ;胃窦 :1 80± 0 .49cpm、2 85± 0 .6 3cpm、2 98± 0 .2 8cpm ,P <0 .0 1) ;正常慢波百分比值 ,前者也明显降低 (胃体 :41 0 7± 12 .6 7%、5 9 5 7± 19.41%、6 8 49± 1.95 % ;胃窦 :31 84± 15 .6 7%、6 0 5 8± 2 1 5 9%、6 7 19± 4.34% ,P <0 .0 1) ;并且Hp +NUD患者胃电节律紊乱发生率亦显著多于后两者 (胃体 :37 5 %、18 2 %、11 1% ;胃窦 :45 8%、18 2 %、0 ,P <0 .0 1)。结论　Hp +NUD患者胃电节律紊乱发生率明显高于Hp -NUD患者及健康人 。Aim To investigate the effects of Hp on cutaneous EGG in NUD.Methods The biopsy specimens were used for Hp status by rapid urease stain and Giemsa stain,and the relationship was analyzed between Hp and cutaneous EGG in NUD.Results 35 patients with NUD,24 were Hp positive (Hp+),and 11 patients were Hp negative(Hp-).APs in Hp+ group were much higher than that of Hp-and healthy subjects (HS)(269.61±81.11μv、148 10±61 32μv、156 75±71 23μv,respectively in body;272.94±68.95μv、150 41±71 11μv、193 50±98 74μv,respectively in antrum, P <0.01);but FP in HP+group were much lower than those of the latters(1 77±0.36cpm、2.80±0.77cpm、2.93±0.38cpm,respectively in body;1.80±0.49cpm、2.85±0.43cpm、2.98±0.28cpm,respectively in antrum, P <0.01);Percent of normal slow wave in Hp+group were also much lower than those of the latters(41.07±12.67%、59.57±17.45%、68 49±1 95%,repectively in body;31.84±15.67%、60.18±21.59%、67 19±4.34%,respectively in antrum, P <0.01)and the prevalence of gastric dysrhythmias in Hp+group were significantly higher than that of Hp-and HS groups(37.5%、18 2%、11 1% in body;45.8%、18 2%、0 in antrum, P <0.01).Conclusion Patients with Hp infection have a higher prevalence of gastric dysrhythmias than that without Hp infection and HS group,which suggested Hp may result in the dysruption of the normal gastric slow wave in NUD.
