摘  要:目的分析35岁及以下青年乳腺癌患者的临床病理特征及相关生物学指标表达情况。方法回顾性分析50例35岁及以下青年乳腺癌病例(青年组)的临床病理特征包括病理类型、病理分级、肿瘤大小、淋巴结转移、TNM分期。免疫组化检测雌激素受体(ER)、孕激素受体(PR)、HER-2、p53、Ki67的表达情况。随机抽取同期35岁以上非青年乳腺癌50例(非青年组)作为对照进行比较。结果青年组以浸润性导管癌为主(占84.0%),腋窝淋巴结转移率明显高于非青年组(58.0%vs.36.0%,P〈0.05),ER表达阳性率低于非青年组(44.0%vs.64.0%,P〈0.05),p53、Ki67表达阳性率高于非青年组(66.0%vs.46.0%,88.0%vs.72.O%,P〈0.05)。2组间PR、HER-2表达阳性率和1年无病生存率差异均无统计学意义(P〉0.05)。结论青年乳腺癌侵袭性强,多项生物学指标联合检测对其预后判断及治疗有重要意义。Objective To explore the clinicopathological characteristics and expression of biomarkers in breast canc- er patients under the age of 35. Methods 50 women aged 35 years or younger with breast cancer (younger age group) were retrospectively evaluated for the clinicopathological characteristics ( histological type and grade, tumor size, lymph node metastasis, TMN staging). An immunohistochemical analysis was performed to determine estrogen receptor (ER) and progesterone receptor (PR) expression and HER -2, p53, Ki67 expression. 50 women aged over 35 years with breast cancer ( non - younger age group) were randomly selected and served as control. Results Invasive ductal carci- noma was the most common histological type in breast cancer patients aged 35 years or younger ( 84.0% ). Younger age group had a higher lymph node metastasis rate than non - younger age group ( 58% vs. 36% ,P 〈 0.05 ). Lower ER pos- itive expression rate (44.0% vs. 64.0% , P 〈 0.05 ) and higher p53 and Ki67 positive expression rates ( 66.0% vs. 46. 0% and 88.0% vs. 72.0% , P 〈 0.05 ) were observed in younger age group compared to non - younger age group. There was no statistically significant difference in PR and HER - 2 positive expression rates and 1 year disease - free survival rate between the 2 groups (P 〉 0.05 ). Conclusion Younger breast cancer patients present with more aggressive tumor characteristics. Combined detection of biomarkers has practical value in the evaluation of prognosis and therapeutic effects on breast cancer of younger patients.
