摘 要:目的研究无痴呆血管性认知障碍(VCIND)患者的磁共振影像学特点,并分析其与神经心理评分的关系。方法所有研究对象均进行一般检查、神经心理评分及头颅磁共振等检查。结果 VCIND组多发病灶者发生率64.44%,较对照组的39.29%明显增高(χ2=4.411,P=0.036);病灶位于额颞叶的比例(48.89%)较对照组(25.0%)也明显增高(χ2=4.114,P=0.043)。VCIND组多发病灶病例较单发病灶病例的MMSE及MoCA评分减少(t=-2.15,P=0.04及t=-2.55,P=0.01),病变位于额颞叶者较位于其他部位者MMSE及MoCA评分减少(t=-2.63,P=0.01及t=-2.83,P=0.01)。结论磁共振显示的病灶数量及部位对VCIND患者认知功能有一定影响,数量多、位于额颞叶者认知功能损害更加明显。MMSE及MoCA评分与VCIND患者的影像学改变具有一定的相关性。Objective To investigate the MRI changes and its image features in vascular cognitive impairment no dementia(VCIND) patients,and to analyze its relationship with neuropsychological score in the meantime.Methods Forty-five cases of VCIND group(male of 25 cases,female of 20 cases) were recruited and 28 stroke cases of non-VCIND(male of 16 cases,female of 12 cases) were recruited as control group.All subjects underwent a general examination,neuropsychological score,and a brain magnetic resonance scan.Results The proportion of multiple lesions occurred(64.44%),which was significantly higher(χ2=4.411,P=0.036) compared with the control group(39.29%).The proportion of lesions located in the front-temporal lobe(48.89%) was significantly higher(χ2=4.114,P=0.043) compared with the control group(25.0%).Within the VCIND group,the MMSE and MCoA score reduced significantly(t=-2.15,P=0.04;t=-2.55,P=0.01) in the cases of multiple lesions,compared with the cases of single lesion;the MMSE and MCoA score also reduced significantly(t=-2.63,P=0.01;t=-2.83,P=0.01) in the cases with the lesions of the front-temporal lobe than the other parts.Conclusion The numbers of lesions and their locations which magnetic resonance show a certain degree of effect on cognitive function in VCIND patients,the multiple lesions and lesions located in front-temporal lobe could due to cognitive impairment obviously,MMSE and MCoA score have a certain degree of correlation with the change of magnetic resonance imaging.
