摘  要:目的探讨放射性核素骨显像(ECT)联合神经元特异性烯醇化酶(NSE)、Ι型胶原交联氨基末端肽(NTx)、骨涎蛋白(BSP)诊断肺癌骨转移的价值。方法随机选取行ECT的肺癌患者82例为实验组,根据是否发生骨转移分为骨转移组(≤2个骨转移灶,>2个骨转移灶)和无骨转移组;另选30例健康体检者为对照组,应用罗氏Cobas601全自动化学发光仪、ELISA法分别检测外周血清中NSE、BSP、NTx的含量。结果实验组血清中NSE、BSP、NTx含量明显高于对照组(t=6.47、4.92、6.88;P<0.01)。实验组中ECT显示发生骨转移45例(骨转移组),显像正常无骨转移37例(无骨转移组),骨转移组与无骨转移组的基本情况比较差异无统计学意义(P>0.05)。无骨转移组NSE、NTx、BSP的含量均高于对照组,差异无统计学意义(t=0.8、1.28、1.34,P=0.43、0.20、0.18);骨转移组血清相关指标含量明显高于对照组(t=9.13、9.47、9.99,P<0.01)。小于或等于2个骨转移灶患者的NSE、BSP、NTx水平明显低于大于2个骨转移灶患者的血清相关指标水平(t=2.28、2.61、2.65,P=0.028、0.012、0.014)。ECT联合外周血清相关指标含量检测的敏感性和特异性可以达到98.3%和95.5%。结论 ECT联合血清中NSE、BSP、NTx水平检测可以提高肺癌骨转移的检出率。Objective To explore the value on ECT combined with detection of NSE,NTx,BSP in diagnosis of bone metastasis for lung cancer.Methods Random select the row of 82 cases of patients with lung cancer ECT for the experimental group,according to whether happen bone bone metastases are divided into groups(≤2 bone metastases,2 bone metastases),and no bone metastasis group;The other 30 cases of healthy physical examination person as control group,the application of Roche Cobas601 automatic chemiluminescence meter,ELISA method measured the level of serum NSE、BSP、NTx.Results Serum NSE、BSP、NTx levels in experimental group were significantly higher than those in controls(t=6.47,4.92,6.88;P0.01);ECT show bone metastasis 45 cases,normal imaging 37 cases,the basic situation of the bone metastasis group and no bone metastasis group is no statistically significant differences(P0.05);The serum tumor marker levels in patients without osseous metastasis were only insignificantly higher than those in controls(t=0.8,1.28,1.34,P=0.43,0.20,0.18).The serum levels of NSE,BSP,NTx in patients osseous metastasis were significantly higher than those in controls(t=9.13,9.47,9.99,P0.01).the levels in patients with more than 2 bone lesions were significantly higher than those in patients with only one or two bone lesions(t=2.28,2.61,2.65,P=0.028,0.012,0.014).ECT joint correlation markers detection sensitivity and specificity is 98.3% and 95.5%.Conclusion ECT combined with NSE,NTx,BSP can improve the detection rate of bone metastasis for lung cancer.
