宫颈癌相关宫旁韧带内淋巴定性定量研究

摘　　要：目的探讨宫颈癌患者子宫主韧带、骶子宫韧带、阴道旁组织中淋巴的含量及分布。方法收集2008年4月至2010年8月在南方医科大学南方医院等3家医院广泛性子宫切除术术后主韧带、骶子宫韧带、阴道旁组织新鲜标本各22条、29条、28条,将标本分为近、中及远3段;主韧带标记浅、深两层,骶子宫韧带分别标记浅、深两层及内、外侧面。对所有标本分别采用HE染色、D2-40染色进行定性分析后,采用Impro图像分析软件和生物体视学对其进行定量分析。结果定性研究:各韧带均可见特异标记淋巴的抗体D2-40染色阳性,HE染色进一步显示了散在分布的淋巴管、粗细不等的各级淋巴管分支,偶见淋巴结。定量研究:主韧带、阴道旁组织中淋巴呈散在分布;骶子宫韧带中淋巴相对集中分布于骶子宫韧带内侧面(P<0.01),且近子宫段最多(P<0.05)。结论宫颈癌患者宫旁组织内淋巴的分布有一定规律,其分布特点对指导术中对宫旁组织的正确处理具有重要作用。Objective To determine the distribution and quantity of lymphatic in the cardinal ligament, uterosacral ligament and paracolpium of cervical cancer. Methods Fresh specimens （ cardinal ligament ： 22 pieces, uterosacral ligament： 29 pieces; paracolpium：28 pieces） were collected from patients who had underwent radical hysterectomy, and cut into near, middle and far segment. The cardinal ligament, uterosacral ligament were subdivided into superficial and deep part from the middle in which facies medialis and facies lateralis were also subdivided when analyzed the uterosacral ligament. Lymphatic vessel were quantitative analysis using Impro image analysis software and biosystem stereology after HE, D2-40 Immunohis- tochemistry staining. Results Qualitative research： D2-40 staining identified the existence of the lymphatic in three liga- ments, and HE staining shows the lymphatic vessel, the branch of the each lymphatic vessel. Rare lymphatic nodus was found. Quantitative research ： Lymphatic were scattered in the cardinal ligament and paracolpium. The Vv of the lymphatic in the uterosacral ligament was concentrated in the facies medialis （ P 〈 0.01 ）, and it was the most in the near segment （P 〈 0.05 ）. Conclusion The distribution of the lymphatic in three ligaments has a certain regular. The characteristic of the lymphatic plays an important role in the surgery when deal with the tissues around the cervix.
