左乙拉西坦添加治疗青年难治性部分性癫痫患者的效果及对认知功能的影响

摘　　要：目的:评价新型抗癫痫药物左乙拉西坦添加治疗青年难治性部分性癫痫患者的疗效,及其对认知功能的影响。方法:30例确诊为难治性部分性癫痫的青年患者随机分为左乙拉西坦组和吡拉西坦组,观察治疗的有效率;利用蒙特利尔认知评估(MOCA)量表评价两组患者在左乙拉西坦剂量维持期(12周)和开放性治疗期(12周)的认知功能。结果:治疗12周末,左乙拉西坦组治疗有效率(75.0%)高于吡拉西坦组(35.7%;Fisher精确概率法:P=0.003),认知功能明显改善(与基线比较:t=3.905,P=0.001)。治疗24周末,两组患者认知功能较治疗前均显著改善(左乙拉西坦组:t=4.940,P=0.021;吡拉西坦组:t=2.575,P=0.023)。结论:左乙拉西坦作为抗癫痫药的添加治疗药物,能够显著减少青年难治性部分性癫痫患者的发作频率,同时可以部分改善癫痫患者的认知功能。Objective To evaluate the therapy efficacy of levetiracetam, which was one of the neotype antiepileptic drugs （AEDs）, on cognitive function in young patients with refractory partial epilepsy. Methods Thirty young patients with refractory partial epilepsy were randomized into 2 groups, the treatment-group and the control-group. The therapy efficacy and the cognitive function evaluated by MOCA scale of patients in 2 groups was compared after 12 weeks hnd 24 weeks therapy, respectively. Results The therapeutic efficacy in the treatment group was significantly higher than that in the control group （75.0% vs 35.7%; P = 0.003）. In comparison to the baseline, the cognitive function was significantly improved after 12 weeks treatment with levetiracetam （t = 3.905, P = 0.001）. The cognitive function of patients in 2 groups were significantly improved after 24 weeks in comparison to the baseline （t = 4.940, P = 0.021 ; t = 2.575, P = 0.023）. Conclusion Levetiracetam has therapeutic efficacy for young patients with refractory partial epilepsy as an adjunctive treatment drug. Levetiracetam could significantly decrease the seizure frequency and improve the cognition function of epileptic patients.
