手法劈核在硬核白内障小切口摘出术中的应用

摘　　要：目的探讨硬核白内障手法劈核方法及临床效果。方法对172例（172眼）IV～V级硬核的白内障行5．5mm带球结膜瓣的角巩膜隧道式切口，将核劈分为2—3块后摘出，植入5．5mm直径的硬质人工晶状体。观察术中术后并发症及术后视力。结果环形撕囊成功160例，成功率93．0％；后囊破裂5例，占2．9％；均植入人工晶状体，植入率100％。术后1d视力≥0．3者136眼，占79．1％；术后7d视力≥0．3者148眼，占86．0％。术后角膜轻中度水肿59眼，占34．3％，术后1—2周完全消退。结论经5．5mm角巩膜隧道式切口手法劈核摘出术可以明显减小切口大小，提高手术疗效，是一种效果好，较为安全的手术方法。Objective To investigate the surgical technique and clinical effect ot manual chop lor hard nucleus cataract. Methods 172 cases （ 172 eyes） of IV - V grade hard nucleus cataract were ob- served. 5.5 mm scleral tunnel incision in limbus, the nucleus were splited into 2 - 4 blocks to be extracted and implantation of 5.5mm diameter rigid intraocular lens. The intraoperative and postoperative complica- tions and postoperative visual acuity were observed. Results Circular capsulorhexis was successful in 160 cases, and the success rate was 93% ; posterior capsular rupture in 5 cases （2.9%） , intraocular lens were implanted in all patients （ 100% ）. The visual acuity was 0.3 or better in 136 eyes （79.1%） at 1 day after surgery. The visual acuity was ≥0.3 in 148 eyes（86% ） at 7 days after surgery. Mild to moderate postoperative corneal edema in 59 cases（34.3% ）, subsided completely at 1 - 2 weeks postoperatively. Conclusion The 5.5 mm scleral tunnel incision combined with manual nucleus chop can obviously reduce the incision size, improve the effect of operation, is a kind of effective, safe operation method.
