胃腺癌组织中紧密连接蛋白7和紧密连接蛋白18及E钙黏素的表达及其意义

摘　　要：目的探讨胃腺癌组织中紧密连接蛋白7(claudin-7)、紧密连接蛋白18(claudin-18)及E钙黏素(E-cadherin)的表达及其意义,为肿瘤生物学研究提供参考。方法选择我院2005年收治的胃腺癌患者81例,收集其手术切除后存档的腺癌蜡块组织为观察组。选择同期在我院进行胃镜检查正常者70例,收集其胃体正常黏膜组织为对照组。应用免疫组织化学法检测两组claudin-7、claudin-18和E-cadherin的表达,分析三者阳性表达率与胃腺癌患者临床特征的关系及三者之间的相关性。随访并计算观察组患者5年生存率。结果对照组和观察组claudin-7(89.33%vs.25.93%)、claudin-18(64.29%vs.33.33%)和E-cadherin(57.14%vs.22.22%)阳性表达率比较,差异均有统计学意义(P<0.05)。claudin-7、claudin-18和E-cadherin阳性表达率在不同肿瘤分化程度、有无淋巴结转移及不同增殖细胞核抗原表达的胃腺癌患者中比较,差异均有统计学意义(P<0.05)。线性相关分析显示,胃腺癌组织中claudin-7与claudin-18(r=0.43)、claudin-18与E-cadherin(r=0.45)、claudin-7与E-cadherin(r=0.40)均呈正相关(P<0.05)。观察组患者5年生存率claudin-7阳性表达者(14/21,66.67%)与阴性表达者(3/60,5.00%)比较、claudin-18阳性表达者(16/27,59.26%)与阴性表达者(1/54,1.85%)比较、E-cadherin阳性表达者(14/18,77.78%)与阴性表达者(3/63,4.69%)比较,差异均有统计学意义(χ2值分别为24.5880、14.7868和13.2048,P均<0.05)。结论 claudin-7、claudin-18和E-cadherin在胃腺癌组织中呈低表达,与患者肿瘤分化程度、淋巴结转移及增殖细胞核抗原表达相关;三者具有协同作用,联合检测三者的表达有助于判断患者预后,即claudin-7、claudin-18和E-cadherin同时低表达的患者预后较差。Objective To detect the expressions and significance of claudin-7,claudin-18 and E-cadherin in gastric adenocarcinoma in order to provide evidence for the study of oncobiology. Methods Adenocarcinoma tissues of 81 patients with gastric adenocarcinoma were collected as observation group,and normal mucosa tissues of 70 normal people underwent physical examination in our hospital in the same period were collected as control group.Immunohistochemical method was used to detect the expressions of claudin-7,claudin-18 and E-cadherin in the two groups.The positive expression rate of the three indicators and their relationship with clinical characteristics were analyzed.The patients were followed up and five-year survival rate was calculated. Results The positive expression rate of claudin-7（89.33%vs.25.93%）,claudin-18（64.29%vs.33.33%）and E-cadherin（57.14%vs.22.22%） between the two groups all showed statistically significant differences（P0.05）.The positive expression rates between patients with different level of differentiation,lymph node metastasis and PCNA expression all showed statistically significant differences（P0.05）.Single factor linear correlation analysis showed that there were positive correlation between claudin-7 and claudin-18（r=0.43）,claudin-18 and E-cadherin（r=0.45）,and claudin-7 and E-cadherin（r=0.40）.The five-year survival rate showed statistically significant differences between positive claudin-7 cases（14/21,66.67%）and negative claudin-7 cases（3/60,5.00%）（χ2=24.5880,P0.05）,between positive claudin-18 cases（16/27,59.26%）and negative claudin-18 cases（1/54,1.85%）（χ2=14.7868,P0.05）,and between positive E-cadherin cases（14/18,77.78%）and negative E-cadherin cases（3/63,4.69%）（χ2=13.2048,P0.05）. Conclusion The expressions of claudin-7,claudin-18 and E-cadherin are low in gastric adenocarcinoma tissues and are related to differentiation level,lymph node metastasis and PCNA expression.The three indicators have synergistic effect and combined
