摘  要:目的观察选择性股内侧肌训练改善脑卒中患者膝过伸的临床疗效。方法将符合入选标准的68例脑卒中膝过伸患者随机分为治疗组31例和对照组37例,均给予药物及常规康复治疗,对照组配合膝关节常规康复训练,治疗组在此基础进行选择性股内侧肌训练,均持续8周。比较分析治疗前后两组患者膝过伸改善情况、Fugl-meyer(FMA)评分及最大步行速度和步长的测量结果。结果治疗8周后治疗组膝过伸3例,对照组膝过伸11例,两组比较差异有统计学意义(χ2=5.080,P<0.05)。治疗前两组下肢FMA评分、最大步行速度及平均步长比较,差异均无统计学意义(P>0.05);治疗8周后两组FMA评分、最大步行速度及平均步长比较,差异均有统计学意义(P<0.01)。结论选择性股内侧肌训练能明显改善脑卒中患者膝关节功能,提高其步行速度。股内侧肌肌力下降可能是膝关节动态稳定性异常的重要因素。Objective To investigate the efficacy of selective vastus medialis oblique exercises on knee hyperextension of stroke patients. Methods 68 stroke patients with knee hypertension were randomly divided into treatment group(31 cases) and control group(37 cases).Both groups were given routine rehabilitation therapy.The control group was given routine knee rehabilitation exercises while the treatment group was given additional selective vastus medialis oblique exercises for 8 weeks.The knee hypertension improvement,Fugl-meyer Score(FMA),maximum walking speed(MWS) and stop length(SL) were compared between the two groups before and after treatment. Results After 8 weeks treatment,3 cases in treatment group and 11 cases in control group had knee hypertension,and the difference was statistically significant(χ2=5.080,P0.05).The FMA score,MWS and SL between the two groups showed no statistically significant differences before treatment(P0.05),but showed statistically significant differences after 8 weeks treatment(P0.01). Conclusion Selective vastus medialis oblique exercises can significantly improve knee function and MWS in stroke patients.Muscle weakness in vastus medialis may be an important factor for the abnormal knee dynamic stability.
