77例帕金森病患者的情绪障碍分析

摘　　要：目的探讨帕金森病患者的情绪障碍、影响因素及心理干预模式。方法从我院神经内科100例原发性帕金森病患者中筛选出77例情绪障碍患者,随机数字表法分成对照组和研究组进行研究,并对可能引起情绪障碍的影响因素进行多因素分析。结果通过认知疗法干预后,研究组HAMA、HAMD总分及各因子分均较对照组显著降低,可得出情绪障碍的产生与认知不足有关;肌强直、姿势步态障碍等临床表现以及病程、家庭经济情况是产生情绪障碍的关键因素。结论患者对帕金森病认知不足易产生情绪障碍,临床症状、病程、家庭经济情况是情绪障碍产生的关键原因,药物治疗配合心理干预可以达到更好的疗效。Objective To explore the emotional disorder factors and psychological intervention model in patients with Par- kinson＇s disease. Methods Totally 77 patients with mood disorders were selected from 100 patients with primary Parkinson＇s disease in the Affiliated Hospital of Putian College by HAMA and HAMD. They were randomly divided into control group and study group to study the impact factors lead to emotional disorders, and multivariate analysis was used. Results Through cog- nitive therapy intervention, the total score and factor scores of HAMA, HAMD in the study group were significantly lower than those in the control group, emotional and cognitive disorders related to lack of knowledge about Parkinson＇s disease; myo- tonia, gait disorder and course of disease, the economic situation were the key factors in emotional disorders. Conclusion Lack of knowledge in patients with Parkinson＇s disease is easy to produce emotional disorders. Clinical symptoms, course of disease, the economic situation are the key reasons of emotional disorders. Drug treatment with psychological intervention is beneficial to Parkinson＇s disease.
