糖皮质激素治疗葡萄膜炎的临床效果观察

摘　　要：目的探讨葡萄膜炎的治疗方法和临床效果。方法于2006年1月～2011年1月使用糖皮质激素治疗葡萄膜炎78例(103眼)。随访观察6个月以上。结果葡萄膜炎按病变位置分型:前葡萄膜炎47眼(45.63%),中间型葡萄膜炎5眼(4.85%),后葡萄膜炎20眼(19.42%),全葡萄膜炎31眼(30.10%)。经治疗后除1眼没有视力增加外,其余病例视力均有不同程度的增加。其中视力≥0.5者由治疗前的47眼(45.63%),增加为治疗后的74眼(71.84%)。并发症包括:虹膜后粘连45眼(43.69%),晶状体混浊31眼(30.10%),玻璃体浑浊28眼(27.18%),视网膜病变27眼(26.21%),青光眼9眼(8.74%)等。结论葡萄膜炎伴有多种并发症的发生,针对不同的病情采取个体化的治疗方案可以减轻药物的毒副作用,能更有效地治疗葡萄膜炎。Objective To discuss the therapeutic methods of uveitis and its clinical effects. Methods Seventy-eight cases （103 eyes） of uveitis were treated by Glucocorticoid from January 2006 to January 2011 with more than 6 months follow up. Results According to pathotype, 47 eyes of anterior uveitis （45.63%）, 5 eyes of intermediate uveitis （4.85%）, 20 eyes of posterior uveitis （19.42%） and 31 eyes of panuveitis （30.10%） were included in our study. All the other cases have visual acuities increased in varying degrees except for 1 eye in a case after Glucoeorticoid treatment. 74 eyes （71.84%） after treatment have visual acuities not least than 0.5 compared with 47 eyes （43.69%） before treatment. The uveitis common complications include： posterior syneehia of 45 eyes （43.69%）, clouding of the lens of 31 eyes （30.10%）, vitreous opacity of 28 eyes （27.18%）, retinopathy of 27 eyes （26.21%）, glaucoma of 9 eyes （8.74%）, etc. Conclusion Uveitis have different complications, we can achieve less toxic side effects and better clinical effects by personalized treatment concerning different cases.
