抑郁症相关情绪疾病中医证候要素与症状相关性的古代文献研究

摘　　要：目的:通过古代文献研究,探讨郁证的中医证候要素与症状的相关性,为规范郁证证候特征文献研究提供依据。方法:利用中华大型电子图书《中华医典》,检索民国以前中国历代医学古籍1000余部,检索词包括"郁证"、"百合病"、"脏燥"、"梅核气",经统计分析找出与上述病证相关的中医证候要素与症状相关性。结果:共纳入851篇文献的内容(851条原文),2873个症状,规范后185个症状,18个证候要素,对证候要素与症状的相关性进行统计,结果显示:气滞与情绪抑郁、烦躁、吞酸、胁肋胀痛等相关;气虚与悲伤欲哭、表情淡漠、反应迟钝、胆怯易惊等相关;血虚与健忘、失眠、头痛、头晕等相关;内火与悲伤欲哭、乏力、烦躁、口苦等相关;痰与悲伤欲哭、表情淡漠、反应迟钝、咽中异物感等相关;肝与烦躁、口干、情绪抑郁、头晕等相关;脾与表情淡漠、反应迟钝、乏力、倦怠等相关;心与胆怯易惊、健忘、哭笑无常、失眠等相关;肾与乏力、口干、小便不利、肢体酸软等相关。结论:古代文献中记载的郁证证候特征与目前大多数医家对郁证的认识基本相符,古代文献中对郁证的论述可为当代研究抑郁症提供良好的借鉴和帮助。Objective：This research seeks the correlations between the syndrome elements and symptoms through the ancient literatures in order to provide the basis for regulating the Yuzheng features in the literature research.Methods：This study searched more than 1000 medical books before Republic of China using the large electronic book Chinese Medical Classics,index words including Yuzheng,Baihebing,Zangzao and Meiheqi,through the statistical analysis to find the correlations between disease and syndrome elements.Results：The contents of 851 articles included（851 original articles）, 2873 symptoms,185 symptoms after standardizing,18 syndrome elements.The results show that after statistical correlation： there are correlations between stagnation of Qi and depressed mood,dysphoria,acid regurgitation,distending pain in hypochondrium;deficiency of Qi and sadness to cry,apathetic facial expressions,lags in response,timid;blood deficiency and amnesia,insomnia,headache,dizziness;endogenous fire and sadness to cry,feeble,dysphoria,bitter taste;phlegm and sadness to cry,apathetic facial expressions,lags in response,foreign body sensation in pharynx;liver and dysphoria, dry mouth,depressed mood,dizziness;spleen and depressed mood,dysphoria,feeble,tireness;heart and timid,amnesia, cry and laugh uncertain,insomnia;kidney and feeble,dry mouth,dysuria,soreness and weakness of limbs.Conclusion： They are mostly consistent in Yuzheng between the ancient literature records and the most understanding in current,so the discusses of ancient literatures can provide good reference and help for contemporary studies.
