超声引导粗针活检在触诊阴性乳腺肿瘤诊断中的临床意义

摘　　要：目的回顾性分析在触诊阴性乳腺肿瘤的诊断和治疗中,超声引导下粗针穿刺活检技术(ultrasound guidecore-needle biopsy,USCNB)的临床应用价值。方法对216例临床触诊阴性,超声诊断高度怀疑为乳腺癌的患者行USCNB,取部分肿瘤组织进行病理组织学检查,结合术后常规病理学检查或术中快速冰冻病理学检查对比分析。结果 USCNB对肿块直径≤0.5 cm的穿刺确诊率小于其他较大肿块者(P<0.05);总体敏感性为87.0%,特异性为91.2%,假阳性率为0。结论 USCNB对乳腺肿块确诊率因肿块大小不同而有差异。USCNB具有定位准确、创伤小、并发症少、操作简便、准确性高的优点,对乳腺癌尤其是触诊阴性的乳腺肿块的定性诊断有较高的临床意义。Objective To analyze the clinical application value of ultrasound guide core-needle biopsy（USCNB）in diagnosis and treatment of palpation negative breast tumor retrospectively.Methods 216 highly suspicious or malignant cases were treated with USCNB in the case of negative palpation.Part of the tumor tissue was examined by histopathology and compared with the routine postoperative pathology examination or fast frozen pathologic examination.Results The rate of correct diagnosis by USCNB in tumor size ≤ 0.5 cm was lower than that in other larger size（P 0.05）.The sensitivity of USCNB in the diagnosis of breast cancer was 87.0%,and the specificity was 91.2%,while the rate of false positives was 0.Conclusion USCNB has accurate positioning and low complications,it also has higher value of clinical application for early breast cancer screening even for that can＇t be palpable.
