旋转式伽玛刀治疗放疗后1年内复发脑转移瘤26例分析

摘　　要：目的:探讨脑转移瘤放疗后复发的治疗方法。方法:对26例放疗后1年内复发脑转移瘤给予旋转式头部伽玛刀治疗。肿瘤边缘以45%～55%等剂量曲线覆盖,周边剂量12～18Gy,中心剂量24～36Gy,1次完成照射,伽玛刀治疗后每隔1、3、6个月后定期电话或返院随诊,同时进行CT或者MRI影像复查,观察患者的临床效果和并发症,统计总有效率。结果:肿瘤消失15例,明显缩小9例,2例无变化,总有效率92.3%。所有病例症状均有所改善,且无明显的不良反应发生。结论:使用旋转式伽玛刀治疗放疗后1年内复发的脑转移瘤是安全有效的。Objective：To investigate the treatment of post-radiotherapy brain metastases recurrence.Method：Twenty-six patients with post-radiotherapy brain metastases recurrence within 1 year were treated with revolving type head gamma-knife radiotherapy.The edge isodose line ranged from 45% to 55% with a circumference dose of 12～18 Gy and the maximal dose ranged from 24 to 36 Gy by single irradiation.After the gamma-knife treatment,regular telephone interview or returned to hospital were followed up after 1,3 and 6 months.CT or MRI scans were taken to observe the clinical effect of patients and detect any complications,evaluate the overall response rate.Result：The tumors disappeared in 15 cases,had significantly narrowed in 9 cases,and unchanged in 2 cases.The total response rate was 92.3%.In all cases the symptoms were improved,with no obvious adverse reaction occurred.Conclusion：Revolving type head gamma-knife radiotherapy is effective and safe in the treatment for post-radiotherapy brain metastases recurrence within one year.
