摘  要:背景与目的:以往3M与5M显示器对乳腺微钙化识读的研究,都是在低分辨率乳腺X线数据情况下完成的。本文通过使用高、低两种分辨率的乳腺影像图来比较3M与5M医用专业显示器对乳腺微钙化诊断效能的影响。方法:选择高、低两种分辨率的乳腺影像各60例(病理证实为癌30例,乳腺病钙盐沉积30例)。由1名高年资和1名低年资放射诊断科医师评估两种显示器对乳腺微钙化的检出率及恶性可能性,检出率采用χ2检验,恶性可能性采用方差分析。结果:微钙化检出率方面:在低分辨率影像图组中,2名医师对乳腺微钙化的检出率差异无统计学意义(P>0.05);而在高分辨率影像图组中,高年资医师使用5M医用专业显示器对乳腺微钙化的检出率明显高于3M(P=0.027),低年资医师的检出率差异无统计学意义(P=0.364)。在微钙化恶性可能性的判读上:无论哪组数据,在5M显示器上,2名医师对微钙化恶性可能性的判读结果均要优于3M显示器,但差异均无统计学意义(P>0.05)。2名医师之间的Az值比较差异无统计学意义(P>0.05)。结论:对不同分辨率的乳腺影像图,应配套相应分辨率的显示器,已便获得最佳的效价比。Background and purpose: In the past, the studies of 3- and 5-megapixel medical professional monitors in detecting micro-calcifications were based on low-resolution breast images. Our study was to evaluate the influence of 3- and 5-megapixel medical professional monitors in detecting micro-calcifications on high- and low- resolution breast images. Methods: A retrospective study was performed in low- and high-resolution data with 60 cases (30 carcinomas and 30 mastopathia with calcium deposition). Two doctors (junior and senior, respectively) estimated the detection rate of the micro-calcifications and the malignant possibility, without knowing the clinical data and histology. The detection rate of them was assessed by chi-square test, and the malignant possibility was tested by analysis of variance. Results: In the detection rate of the micro-calcifications, the two doctors' detection performance of breast micro-calcifications was very similar in the 3M and 5M medical professional monitors (P〉0.05) in the low resolution data group, while in the high resolution group, the senior doctor's detection rate on the 5M monitor was significantly higher than that on the 3M (P=0.027), while the junior's detection rate had no significant difference (P=-0.364); In the interpretations of malignant possibility of the micro-calcifications, the two doctors' interpretations were superior on 5M monitors to the 3M ones, but the difference was not statistically significant (P〉0.05). And the Az values between the two doctors also had no significant difference (P〉0.05). Conclusion: Different breast images of different resolutions should match corresponding resolution monitor in order to get the best potency ratio.
