摘  要:目的探讨血管内皮生长因子(VEGF)在血液和胰腺组织中的表达水平与胰腺微循环障碍加重过程中的关系及意义。方法健康雄性SD大鼠100只,随机分为对照组(N、32只)、急性水肿型胰腺炎组(AEP、33只)及急性坏死型胰腺炎组(ANP、35只)。检测各组及各时点血中VEGF、血淀粉酶(AMY)和胰腺组织中VEGF表达,观察胰腺病理改变。结果模型建立后,各组随机抽取8只大鼠,测量6、12、24h时点VEGF含量,其中ANP组分别为(119.69±26.80)、(145.78±22.11)、(160.05±21.11)ng/L。AEP组分别为(99.14±29.08)、(115.00±17.04)、(134.04±27.93)ng/L,N组分别为(36.09±8.59)、(46.93±8.93)、(42.06±11.74)ng/L。ANP组及AEP组血清VEGF表达均显著高于N组(P〈0.05);ANP组与AEP组之间差异有统计学意义(P〈0.05);各组胰腺组织中6、12、24h各时点VEGF染色评分ANP组为11.13±1.46、7.25±0.89、5.88±1.25,AEP组为8.53±0.87、10.27±0.83、11.12±0.83,N组为3.50±0.93、2.25±1.04、1.75±0.71。AEP与ANP组胰腺组织中各时点VEGF染色评分均高于N组(P〈0.05);6h-ANP组的评分显著高于AEP组(P〈0.05),而12h-ANP组与24h-ANP组的染色评分明却低于AEP组(P〈0.05)。结论在胰腺微循环障碍持续加重期,胰腺组织中VEGF表达减少,预示胰腺坏死难以逆转。Objective To investigate the relationship between the expression of vascular endothelial growth factor (VEGF) in serum and pancreas tissue and the aggravating course of microcircnlation disorder in the pancreas. Methods 100 male SD rats were randomly divided into three groups: sham normal (SN) group (n = 32), acute edema pancreatits (AEP) group (n = 33) and acute necrosis pancreatits (ANP) group (n =35). At 6, 12 and 24 h after modeling, the serum VEGF, and AMY were measured by enzyme linked immunosorbent assay ( ELISA), the expression of VEGF protein in the pancreas was detected by using immunohistochemistry, and histological changes in the pancreas were examined under a microscope. Results Eight rats were randomly selected after modeling in each group, and the VEGF content at 6, 12, and 24 h was measured. The VEGF contents were ( 119. 69±26. 80), ( 145.78 ± 22. 11 ) and (160.05 ±21.11) ng/L in ANP group, (99. 14 ±29.08), (115.00 ±17.04) and (134.04 ±27.93) ng/L in AEP group, and (36. 09±8.59), (46. 93 ±8. 93) and (42.06 ±11.74) ng/L in SN group, respectively. The VEGF expression in serum of ANP group and AEP group was significantly higher than in SN group ( P 〈 0. 05 ) , and there was statistically significant difference between ANP group and AEP group (P 〈0.05).
