老年急性白血病患者内皮祖细胞的变化及其临床意义

摘　　要：目的探讨老年急性白血病(AL)患者骨髓及外周血中内皮祖细胞(EPCs)的数量变化及其临床意义。方法采用流式细胞仪(FCM)检测66例初治AL患者骨髓及外周血中的EPCs,其中老年AL组28例,非老年AL组38例。以10例良性血液病患者为对照组。结果 (1)老年AL患者骨髓EPCs绝对计数较外周血明显增高(P<0.001),相对计数较外周血无明显差异(P>0.05)。(2)老年AL患者、非老年AL患者骨髓及外周血EPCs水平较对照组明显增高(P<0.01)。老年AL组骨髓及外周血中EPCs水平较非老年AL组降低,两者无统计学差异(P>0.05)。(3)老年AL患者外周血EPCs绝对计数与白细胞计数(r=0.815,P=0.027)、β2-微球蛋白(r1=0.709,P=0.043)、LDH(r2=0.827,P=0.026)呈正相关。结论老年AL患者EPCs水平明显增高,EPCs可能与老年AL发病相关。Objective To evaluate the count of endothelial progenitor cells （EPCs） both in peripheral blood （PB） and bone marrow （BM） of elderly patients with acute leukemia （AL）. Methods EPCs were quantified by flow cytometry procedures in 26 elderly AL patients at the time of diagnosis with 40 young and middle-aged AL patients and 10 benign hemopatby patients as control groups. Results The absolute counting of EPCs was significantly higher in BM than that in BP of elderly AL patients （ P 〈 0. 001 ） . While the relative counting of EPCs had no significant difference between in BM and in BP of AL patients （ P 〉 0. 05 ）. The levels of EPCs in both elderly and non-elderly AL patients were significantly higher than those of control group （ P 〈 0. 01 ）. The levels of EPCs in PB and in BM of elderly AL patients were lower than those in non-elderly AL patients, however, it had no significant difference between the two groups （ P 〉 0. 05 ）. EPCs absolute count in PB of elderly AL had positive correlation with WBC （ r = 0. 815, P = 0. 027 ）, β2- MG （ r1 = 0. 709, P = 0. 043 ） and LDH （ r2 = 0. 827, P = 0. 026）. Conclusions EPCs levels are higher in BM and BP of elderly AL patients and maybe correlate with disease status.
