高阶像差对人工晶状体眼屈光地形图高度及调制传递函数作用研究

摘  要:目的分析白内障术后人工品状体眼3ram、4mm、5mm不同瞳孔直径下影响屈光地彤图高度的主要高阶像差,屈光地形图高度与调制传递函数(MTF)的作用关系。方法临床病例自身对照研究。选取2011年8月至2012年3月在天津市眼科医院就诊的年龄相关性白内障术后患者50例(50只眼),行i-Trace视功能分析仪测量全眼高阶像差、MTF曲线、屈光地形图。使用Image—Pro—Plus6.0图形分析软件进行屈光地形图高度(HRM)测量,对HRM与高阶像差及MTF值进行线性相关分析。结果屈光地形图的高度随瞳孔直径增大而增大,MTF值随瞳孔直径增大而减小,3mm瞳孔大小的情况下,彗差均方根值与屈光地形冈的高度正相关(r=0.784,P〈0.05),4mm瞳孔直径下,彗差的均方根值与屈光地形图的高度正相关(,=0.627,P〈0.05),5mm瞳孔直径下屈光地形图高度与球差正相关(,=0.358,P〈0.05)。3mm瞳孔时HRM与MTF(tHOA-15c/d)呈负相关(P〈0.05,r=-0.606);4mm瞳孔时HRM与MTF(tHOA-15c/d)呈负相关(r=-0.335,P〈O.05)5mm瞳孔时HRM与MTF(tHOA)无相关。结论3mm、4mm瞳孔直径下影响屈光地形图高度的主要高阶像差为彗差,球差在瞳孔直径为5mm对屈光地形图高度开始有影响,但同时也会降低MTF值。Objective To analyze higher-order aberration which influence the height of refraction map (HRM) and investigate the relationship between the HRM and modulation transfer function (MTF) in eyes implanted intraocular lens (IOL) at 3ram, 4mm, 5mm pupil diameter. Methods Fif- ty age-related cataract patients who were operated with phacoemulsification and IOL implantation were enrolled. The i-Trace was used to measure the higher-order aberration, MTF, and refraction map. The image-pro-plus 6.0 software was adopted to analyze the HRM. Results The HRM in- creased with pupil diameter, MTF decreased. The HRM was positive correlation with coma (P 〈 0.05, r =0.784) at 3mm pupil diameter. The HRM was positive correlation with coma (P 〈0.05, r = 0.627) at 4mm pupil diameter. The HRM was positive correlation with spherical aberration (P 〈0.05, r =0.358) at 5mm pupil diameter. The MTF (tHOA-15c/d) was negative correlation with HRM ( r=-0.606, P 〈0.05) at 3mm pupil diameter and 4mm pupil diameter ( r =-0.335, P 〈0.05), while there was no correlation at 5mm pupil diameter. Conclusions When pupil diameter is 3mm and 4mm, the main aberration influence HRM is coma. The spherical aberration begins to influence the HRM at 5mm pupil diameter, but it can decline MTF.
