原发性皮肤间变性大细胞淋巴瘤1例

摘　　要：患者女,78岁。左膝关节上出现两个紫红色肿块6个月,破溃7d。皮损病理组织示:真皮内弥漫性淋巴样肿瘤细胞浸润。此种细胞体积较大,核大,异型性明显,病理性核分裂像多见,并见瘤巨细胞。免疫组化:CD30弥漫(+),Ki-6780%(+),CD20散在(+),CD3和CD45RO大部分(+),LCA(+),CD56,TIA-1和ALK均(-)。诊断:原发性皮肤间变性大细胞淋巴瘤。A 78-year-old woman presented with two prunosus tumors above her left knee joint for 6 month, which having been ulcerated for 7 days. Histopathologic examination revealed infiltration in the dermis of diffuse lymphoid cells. The large-size lymphoid cells had large and pleomorphic nuclei with pathological mitosis. Tumor giant cell also was found. Immunohistochemical stains indicated tumor cells showed diffusively positive for CD30, 80 percent positive for Ki67, scatteredly positive for CD20, Mostly positive for CD3 and CD45RO, positive for LCA, but negative for CD56, TIA-1 and ALK. The diagnosis was made as primary cutaneous anaplastic large cell lvmphoma.
