硼替佐米治疗恶性浆细胞病的近期疗效观察

摘　　要：目的:观察硼替佐米治疗恶性浆细胞病的近期疗效和不良反应。方法:恶性浆细胞病患者7例,其中1例原发性浆细胞白血病,1例髓外浆细胞瘤,5例多发性骨髓瘤。均给予硼替佐米为基础的化疗方案治疗。患者分别接受1～4个疗程的治疗。结果:6例患者均在第1疗程治疗后显示反应,其中l例原发性浆细胞白血病患者治疗1疗程后骨髓达完全缓解,5例部分缓解或接近完全缓解;1例因严重感染死亡。不良反应包括血液学不良反应、周围神经病变、疲乏、胃肠道症状及发热。大部分经对症支持治疗后缓解。结论:硼替佐米对于恶性浆细胞病有效,有一定不良反应,但患者耐受良好。Objective：To observe the short-term therapeutic effects and adverse effects of bortezomib in treatment of malignant plasma cell neoplasm.Method：Seven patients with malignant plasma cell neoplasm treated in our hospital were enrolled,including 1 case of primary plasma cell leukemia（PPCL）,1 case of extramedullary plasmacytoma and 4 cases of multiple myeloma.They all received bortezomib based chemotherapy regimens,respectively from 1 to 4 cycles.Result：Six patients achieved responses in the first regimen,including one case of PPCL achieved CR in bone marrow examination,five cases achieved PR or nCR.One died of serious infection.Adverse events included hematologic toxicities,peripheral neuropathy,fatigue,gastrointestinal and fever.They were generally managed with appropriate supportive care.Conclusion：Bortezomib is effective in treatment of malignant plasma cell neoplasm,and patients have a good tolerance to it.
