27例良性食管溃疡分析

摘　　要：目的进一步分析不同原因引起的食管良性溃疡在年龄分布及镜下表现上的特点。方法对2010年10月-2011年9月检出的27例良性食管溃疡进行回顾性分析。结果 27例良性食管溃疡中胃食管反流引起者13例,50岁以上多见,溃疡位于下胸段,斑片状或条状,背景黏膜充血;不明原因引起者10例,50岁以下多见,其中6例位于中胸段,2例位于中下胸段,2例位于下胸段,溃疡类圆形或椭圆形,浅凹,基底不平,周边黏膜略隆起,背景黏膜无明显异常。结论不明原因食管溃疡多见于50岁以下青壮年,对伴有反复口腔溃疡病史,尤其是活检后溃疡明显扩大者,应警惕白塞氏病的可能。Objective To explore the endoscopic and clinical characteristics on the basis of the different causes with benign esophageal ulcer. Methods The clinical data of 27 patients with benign esophageal ulcer from Oct. 2010 to Sep. 2011 were retrospectively reviewed. Results 13 cases were caused by gastroesophageal reflux in 27 patients of benign esophageal ulcer. Many of them were over the age of 50. The ulcers were located in the lower thoracic segments. The shape of the ulcers was spots or stripes and the background mucosa was congestive. 10 patients by unknown causes were less than the age of 50. The ulcers of 6 eases were located in the middle thoracic segments, 2 cases in the middle and lower thoracic segments, 2 eases in the lower thoracic segments. The shape of the ulcers was round or oval, dimple and uneven base. Surrounding mucosa of the ulcers was uplift. Background mucosa of the ulcers was no difference. Conclusion Unexplained ulcers of the esophagus are more common among young adults under 50 years old. It should be wary of Behcet＇ s disease in whom with a history of recurrent oral ulcers and significant expansion of ulcer area after biopsy.
