摘 要:目的探讨核转录因子-κB(NF-κB)和三叶因子3(TFF3)在胃溃疡和胃癌中表达的意义,以及与幽门螺杆菌(H.pylori)感染的关系。方法胃溃疡组(GU)60例,胃癌组(GC)70例,应用免疫组化方法测定TFF3和NF-κBp65的表达,同时检测H.pylori感染情况。结果 GC中NF-κBp65和TFF3表达水平均高于GU(P<0.01)。GC中NF-κBp65表达与TFF3表达呈显著正相关(r=0.350,P=0.003)。GU中H.pylori阳性病例中NF-κBp65、TFF3表达显著高于H.pylori阴性病例(P均<0.05)。H.pylori感染病例中,GC组的NF-κBp65、TFF3表达水平均比GU组高(P均<0.05);H.pylori阴性病例中,GC组的NF-κBp65、TFF3表达水平亦均比GU组高(P均<0.01)。结论 NF-κBp65和TFF3的高表达与胃癌的发生发展有着密切的关系,二者可能存在协同作用。H.pylori感染可以上调宿主胃黏膜NF-κBp65和TFF3的表达,表达的差异与胃溃疡和胃癌两种不同疾病转归相关。Objective To study the expressions of trefoil factor family-3 (TFF3) and nuclear factor kappaB (NF- κB) in human gastric ulcer and gastric cancer, and the correlation with Helicobacter pylori (H. pylori) infection. Methods The expressions of NF-κBp65 and TFF3 were detected by immunohistochemistey in 60 gastric ulcer specimens, 70 gastric cancer specimens. At the same time, the infection of H. pylori was detected. Results The expression levels of NF-κBp65 and TFF3 in GC group were higher than those in GU group (P 〈 0.01 ). There were significantly positive correlations between expressions of NF-κBp65 and TFF3 in GC group (r = 0. 350, P = 0. 003). In GU group, the ex- pression levels of NF-κBp65 and TFF3 in H. pylori positive group were higher than those in H. pylori negative group (P 〈 0. 05). In H. pylori positive group, the expression levels of NF-κBp65 and TFF3 in GC group were higher than those in GU group (P 〈 0. 05). In H. pylori negative group, the expression levels of NF-κBp65 and TFF3 in GC group were higher than those in GU group (P 〈 0.01 ). Conclusion The expressions of NF-κBp65 and TFF3 may be involved in occurrence and development of gastric career. Their expressions are correlated with each other. H. pylori can increase expressions of NF-κBp65 and TFF3 in host gastric mucosa. In the pathogenic process of different gastric diseases, there is a remarkable difference of the expressions of NF-κBp65 and TFF3 activated by H. pylori.
