核基质蛋白22(NMP22)检测对膀胱移行细胞癌早期诊断价值的应用研究

摘  要:目的 探讨核基质蛋白22(rqMe22)的检测对膀胱移行细胞癌(B11Cc)早期诊断的可行性及影响因素。方法采集可疑膀胱移行细胞癌患者、泌尿系良性疾病患者及健康对照人员尿液共225份样本,均行尿NMP22、尿细胞学、尿常规检测及中段尿培养,所有样本均新鲜检测,保存最长不超过4小时。分析比较各种方法的敏感性、特异性、191阳性预测价值和准确性。结果尿液NMP22水平以10.0×103U/L为临界值,超出为阳性,尿细胞学检测以发现肿瘤细胞为阳性,尿常规检测及中断尿培养用于辅助检查及排除其他疾病。尿液NMP22的阳性率为82.3%,与脱落细胞学(20.8%)比较,具有显著性差异(P〈0.01),此两种方法诊断膀胱移行细胞癌的特异性分别为64.4%和100%,阳性预测值分别为54.8%和100%,阴性预测值分别为91.7%和76.2%。结论尿NMP22采样方便,检测技术简单,具有较高的敏感性和特异性,可作为早期诊断膀胱移行细胞癌的肿瘤标志物,但需尽量排除影响因素。Objective To assess the feasibility and influencing factors of urine nuclear matrix protein22 (NMP22) for the early diagnosis of bladder transitional cell carcinoma. Methods 225 subjects, including bladder cancer, urological benign disease and healthy individuals were en- rolled in this study. Urine NMP22 and exfoliocytology examination and routine urinalysis and midstream urine culture were determined in the samples. Analysis and comparison the sensitivity, specificity, positive and negative predictive value of these two ways. Results The sensitivity of NMP22 (82.3 % ) were significantly better than exfolioeytology( 17.7 %, P 〈 0.01 ), the specificity of NMP22 and exfoliocytology were 74.2 % andl00 %, respectively, the positive predictive values were 54.8 % and 100 %, and negative predictive values were 91.7 % and 76.2% . Conclusions NMP22 is a sensitive, specific, feasible and noninvasive diagnostic markers for the early diagnosis of bladder transitional cell carcino- ma, but we should be remove the interference factor first.
