摘 要:目的探讨CEUS评价兔肝VX2移植瘤血流灌注及其在肿瘤血管生成中的价值。方法建立兔肝VX2移植瘤动物模型,之后10~14天进行常规超声及CEUS检查。应用低机械指数CEUS(CnTI)及QontraXt定量分析软件,测得VX2移植瘤和肝实质CEUS定量参数,并与组织病理所示MVD及血管内皮生长因子(VEGF)的蛋白表达水平进行相关性分析。结果成功建立肿瘤模型兔11只,共22个肿瘤。兔肝VX2移植瘤开始增强时间、达峰时间均早于周围肝实质,其峰值强度、曲线下面积、曲线尖度均高于周围肝实质(P均<0.05)。兔肝VX2移植瘤MVD与CEUS的峰值强度呈正相关(r=0.58,P=0.005),与VEGF表达呈正相关(r=0.64,P=0.001)。兔肝VX2移植瘤VEGF的表达均为阳性,肝实质VEGF的表达均为阴性。结论 CEUS可定量反映兔肝VX2移植瘤血流灌注,其峰值强度与MVD呈正相关,可间接提示肝恶性肿瘤的MVD表达水平,评估肿瘤的血管生成。Objective To observe the value of CEUS in tumor blood perfusion and malignant tumor angiogenesis of liver VX2 tumors in rabbits. Methods CEUS was performed 10—14 days after VX2 tumors were inoculated. QontraXt quantitative analysis software was used to analyze the characteristics of tumor blood perfusion, and the perfusion parameters were obtained. MVD and expression of VEGF in tumor were assessed immunohistoehemically, and the correlation with CEUS parameters was analyzed. Results Eleven models with 22 tumors were successfully established. The beginning of time and the peak time of tumors were earlier than those of the surrounding liver parenchyma, and their peak intensity, sharpness and area under curve were significant different with those of peripheral parenchyma (all P〈0.05). There was positive correlation between MVD and peak intensity (r=0.58, P=0.005), as well as expression of VEGF (r=0.64, P=0.001). The expression of VEGF in rabbit liver VX2 tumors was all positive, while of liver parenchyma were negative. Conclusion CEUS can quantitatively reflect the perfusion characteristics and distribution of rabbit liver VX2 tumors. PI of CEUS has positive correlation with MVD in liver VX2 tumors, which may indirectly reflect the level of MVD and therefore be helpful to evaluating tumor angiogenesis.
