胃黏膜病变组织中RASAL_1基因的表达及其与幽门螺杆菌感染的关系

摘　　要：目的探讨RASAL1基因在幽门螺杆菌(Hp)感染胃黏膜组织中的变化及其在胃癌发生发展中的意义。方法内镜及活检病理证实的慢性胃炎58例(慢性胃炎组)、肠化生及异型增生58例(肠化生及异型增生组)和胃癌58例(胃癌组),分别用13C-呼气试验及Giemsa染色检测Hp,链霉菌亲生物素蛋白-过氧化物酶连接(SP)免疫组化检测RASAL1,比较各组及组内Hp阳性亚组和Hp阴性亚组中RASAL1的表达。结果慢性胃炎组、肠化生及异型增生组和胃癌组中RASAL1的阳性率分别为89.66%、56.90%和13.79%,呈递减趋势。各组Hp阳性亚组与Hp阴性亚组中RASAL1的阳性率分别为,慢性胃炎组:73.33%和96.43%,肠化生及异型增生组:40.63%和76.92%,胃癌组:14.29%与13.51%,两两比较均有显著差异。结论 Hp感染可降低胃黏膜RASAL1表达,削弱其对胃黏膜损伤的保护作用,促进了胃癌的发生发展。Objective To explore the changes of RASAL~ gene in gastric mucosa tissue in- fected by helicobacter pylori （Hp） and its significance in the development of gastric cancer. Methods Hp in 58 patients with chronic gastritis confirmed by endoscope and biopsy pathology （chronic gas- tritis group）, 58 patients with intestinal metaplasia and dysplasia （intestinal metaplasia and dysplasia group） and 58 patients with gastric cancer （gastric cancer group） was detected by 13 C urease breath test and Giemsa stain, respectively. RASAL1 was determined by streptomyces biophile protein - strep- tavidin- biotinper- oxidase （SP） immunochemiohistology. The expression of RASAL~ in positive and negative Hp subgroups of each group and within groups was compared. Results The positive rates of RASAL~ gene in chronic gastritis group, intestinal metaplasia and dysplasia group and gastric cancer group were 89.66% , 56.90% and 13.79%, respectively, with a decline trend. The positive rates of RASALI in positive and negative Hp subgroups of each group were respectively 73.33% and 96.43 （chronic gastritis group）, 40.63% and 76.92% （intestinal metaplasia and dysplasia group） as well as 14.29% and 13.51% （gastric cancer group）. Significant differences were observed within groups. Conclusion Hp infection can decrease the expression of RASAL in gastric mucosa, alleviate its pro- tective effect on gastric mucosa and promote the occurrence and development of gastric cancer.
