肝动脉栓塞化疗对肝癌术后复发的预防作用

摘　　要：目的探讨肝动脉插管化疗栓塞(transcatheter hepatic arterial chemoembolization,TACE)对肝癌术后复发的预防作用,为降低肝癌术后复发率提供依据。方法选择作者医院接受手术完整切除的原发性肝癌患者158例,其中84例于术后30天接受TACE,定义为TACE组;其余74例患者未行TACE,定义为对照组。所有患者均通过门诊、电话随访,统计1、2、3年两组患者的累计复发率和累计生存率并进行比较。结果 TACE组随访率95.2%;对照组随访率95.9%。TACE组1、2、3年的累积复发率分别为10.0%、45.0%、67.5%;对照组分别为45.1%、66.2%、84.5%。两组间的差异均具有统计学意义(P<0.05)。TACE组1、2、3年的累积生存率分别为85.0%、72.5%、62.5%,对照组分别为70.4%、56.3%、45.1%。两组间的差异均具有统计学意义(P<0.05)。结论术后TACE有助于尽早发现残癌灶,降低术后复发率,提高患者生存率。Objective To evaluate the effect of transcathater arterial chemoembolization（TACE） in preventing postoperative recurrence of hepatocellular carcinoma and provide evidence to decrease the postoperative recurrence.Methods A total of 158 patients with hepatocellular carcinoma who received complete surgical removal were studied.Eighty-four cases receiving TACE at 30 days after surgery were TACE group and the remaining 74 cases without TACE were control group.All patients were follow-upped in the outpatient department or by telephone.The 1-,2-and 3-year cumulative recurrence rate and 1-,2-and 3-year cumulative survival rate of the two groups were compared.Results The follow-up rate was 95.2% in TACE group,and 95.9% in control group.The 1-,2-,and 3-year cumulative recurrence rates were 10.0%,45.0%,67.5% in TACE group,and 45.1%,66.2%,84.5% in control group.The differences were statistically significant（P〈0.05）.The 1-,2-,3-year cumulative survival rates were 85.0%,72.5%,62.5% in TACE group and 70.4%,56.3%,45.1% in control group.The differences were statistically significant（P〈0.05）.ConclusionPostoperative TACE avails to early detect the residual foci,reduce the recurrence rate and improve patients＇ survival rate.
