摘　　要：目的介绍直肠癌柱状经腹会阴切除(cylindrical abdominoperineal resection,CAPR)及其应用。方法回顾自贡市第二人民医院肛肠科2009年6月至2011年6月CAPR术治疗20例低位直肠癌患者的病例资料。结果 20例行CAPR术患者中发生直肠穿孔1例,病理示环周切缘(CRM)均为阴性,术后腹部切口全部I期愈合,会阴部伤口17例I期愈合,3例感染。术后随访10～34个月,无复发,无盆底疝和腹膜疝。发生会阴疼痛3例,2例发生排尿障碍,4例发生性功能障碍,1例死亡。结论 CAPR术切除了更多癌周组织,消除切除标本狭窄腰部,从而降低了环周切缘阳性率和直肠穿孔率,提高患者生存率。Objective The cylindrical abdominoperineal resection for rectal cancer and its application.Methods Review of second Zigong city the people hospital anus intestines section in 2009June-2011June CAPR for the treatment of 20 cases of patients with low rectal cancer cases.Results 20 cases of CAPR patients occurring in a patient with rectal perforation in 1 cases,pathological circumferential resection margin（CRM） were negative.Postoperative abdominal incision healing of all phase I,perineal wound healing in 17 cases of I,Infection in 3 cases.After follow-up for 10-34 months,no recurrence,no pelvic floor hernia and peritoneal hernia.The onset of perineal pain in 3 cases,2 cases of voiding dysfunction,4 cases of sexual dysfunction,1 cases of death.Conclusion CAPR resection of more cancerous tissue,eliminate the resected specimen narrow waist,thereby reducing the circumferential resection margin positivity and rectal perforation rate,improve the survival rate of patients.
