含左氧氟沙星三联方案治疗幽门螺杆菌阳性消化性溃疡的可行性评价

摘　　要：目的探讨含左氧氟沙星三联方案治疗治疗幽门螺杆菌阳性消化性溃疡的可行性。方法选择2003年1月至2012年3月年幽门螺杆菌阳性的消化性溃疡患者120例,随机分成观察组和对照组两组,每组各60例,对照组应用标准三联疗法:奥美拉唑20mg+阿莫西林1.0g+甲硝唑0.5g,2次/天,口服;观察组奥美拉唑20mg+克拉霉素0.5g+左氧氟沙星0.4g,2次/天,口服,两组共7d一疗程。7d以后继续应用奥美拉唑抗溃疡治疗。结果各疗程结束后1个月复查胃镜和14C-UBT,观察组溃疡治疗总有效率95.00%,幽门螺杆菌根除率93.33%、药物不良反应18.33%,对照组分别为93.33%、78.33%、33.33%。两组比较溃疡治疗总有效率无显著性差异(P>0.05),观察组幽门螺杆菌根除高率、药物不良反应低(P<0.05)。结论左氧氟沙星三联方案根除幽门螺杆菌感染根除率增加,不良反应发生率明显减少,值得临床推广。Objective To investigate and evaluate the feasibility of levofloxacin-containing triple therapy for the treatment of HP positive peptic ulcer.Methods 120 cases of HP positive peptic ulcer from January,2003 to March,2012 were selected and divided into the observation group and control group at random,each group with 60.The control group was treated by the standard triple therapy with the oral administration of 20 mg of omeprazole and 1.0g of Amoxicillin plus 0.5g of metronidazole,twice a day;The observation group was treated with the oral administration of 20 mg of omeprazole and 0.5g of clarithromycin plus 0.4g of levofloxacin,twice a day.After a 7-day course of treatment,both groups applied omeprazole for further treatment of ulcer.Results One month after the course of treatment,each group was reviewed by gastroscope and 14C-UBT.The total response rate of ulcer treatment in observation group was 95.00%,HP eradication rate 93.33%,adverse drug reaction 18.33%,whereas those in the control group were respectively 93.33%,78.33%,33.33%.As the results indicate,there is no striking difference in total response rate of ulcer treatment in both groups（P0.05）,and HP eradication rate in observation group is higher;the rate of adverse reaction is lower（P0.05）.Conclusion Levofloxacin-containing triple therapy significantly increases the eradication rate of HP and obviously reduces the adverse drug reactions;hence,it is worthy of clinical expansion.
