CO_2气腹对人肾癌裸鼠原位移植瘤中HIF-1α、VEGF表达的影响

摘　　要：目的研究腹腔镜CO2气腹对人肾癌裸鼠原位移植瘤中肿瘤增值和转移相关基因和蛋白表达的影响,从而探讨CO2气腹腹腔镜手术治疗肾脏肿瘤的安全性。方法选用人肾透明细胞癌细胞株(786-0),构建BALB/c原位肾癌裸鼠模型40只,根据CO2气腹处理时间不同随机分组:A组(处理0 h),B组(处理2 h),C组(处理4 h),D组(处理4 h后放回鼠笼24 h),后处死裸鼠采取肾癌标本后应用Western blotting和real time RT-PCR检测各组所取标本缺氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)、血管内皮生长因子(vascular endothelialgrow th factor,VEGF)mRNA和蛋白的表达,行统计学分析。结果 B、C组HIF-1α、VEGF mRNA和蛋白相对表达量较A组逐渐增加,B组与A组比无统计学差异(P>0.05),C组与A组比有统计学差异(P<0.01),D组HIF-1α、VEGF mRNA和蛋白相对表达量较B、C组降低,与A组相比无统计学差异(P>0.05)。结论 CO2气腹处理原位肾癌裸鼠,可导致肿瘤细胞HIF-1α、VEGF表达增加,但在4 h内的变化可逆,提示CO2气腹腹腔镜手术治疗肾癌是安全的。Objective To investigate the effects of CO2 pneumoperitoneum on the expression of tumor proliferation and metastasis-related genes and proteins in orthotopically transplanted human renal carcinoma in nude mice.Methods Human renal carcinoma 786-0 cells were orthotopically transplanted in 40 BALB/c nude mice,the animals were randomly divided into 4 groups： no pneumoperitoneum（group A）,CO2 pneumoperitoneum was induced for 2h（group B）,for 4h（group C） and 24h after pneumoperitoneum induced for 4h（group D）.Then,the mice were sacrificed and the specimens of renal carcinoma were collected.HIF-1α and VEGF mRNA and proteins in renal carcinoma 786-0 cells were detected by real time RT-PCR and Western blotting,respectively.Results The mRNA and protein expression levels of HIF-1α and VEGF in group B and C were up-regulated,there were no significant differences between two groups（P0.05）;while the expression in group C was significantly higher than that in group A（P0.01）.The mRNA and protein expression levels of HIF-1α and VEGF in group D were lower than those in group B and C,and were no significant difference to those of group A（P0.05）.Conclusion The expression levels of HIF-1α and VEGF are up-regulated in orthotopically transplanted human renal carcinoma of nude mice during CO2 pneumoperitoneum,but the changes are temporary and reversible,which suggests that laparoscopic surgery with CO2 pneumoperitoneum is a safe procedure for renal carcinoma.
