CEA、NSE及CYFRA21-1联合检测在肺癌诊断与手术治疗中的价值

摘　　要：目的为探讨血清肿瘤标志物癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白片段21-1(CYFRA21-1)联合检测在肺癌鉴别诊断与手术治疗中的意义。方法采用双抗体夹心磁微粒化学发光法分析109例确诊为肺癌患者检测血清中CEA、NSE及CYFRA21-1水平,40例健康者作为对照。结果血清肿瘤标志物CEA、NSE及CYFRA21-1肺癌组测定值分别为(45.6±26.2)ng/mL、(23.8±12.6)ng/mL、(12.7±7.6)ng/mL,显著高于对照组[(4.2±3.3)ng/mL、(8.9±7.4)ng/mL、(1.5±1.2)ng/mL],差异有统计学意义(P<0.01)。单一检测CEA、NSE及CYFRA21-1肺癌的阳性率分别为49.5%、25.6%、52.3%,3种联合检测肺癌的阳性率达80.7%。血清肿瘤标志物CEA、NSE及CYFRA21-1肺癌术后组测定值分别为(28.2±21.3)ng/mL、(17.5±14.3)ng/mL、(8.3±5.7)ng/mL,水平较术前明显减低,差异有统计学意义(P<0.01);与对照组比较,差异有统计学意义(P<0.01)。结论对于不同类型肺癌的术前诊断3种肿瘤标志物联合检测阳性率高于单项检测。手术治疗后1个月CEA、NSE及CYFRA21-1均显著减低,但未降至正常水平。Objective To observe the value of combined detection of CEA,NSE and CYFRA21-1 in diagnosis and treatment of lung cancer.Methods The serum CEA,NSE and CYFRA21-1 of 109 lung cancer patients was detected by double-antibody sandwich chemoluminescence.40 healthy persons were included as control group.Results The serum level of CEA,NSE and CYFRA21-1 of the lung cancer patients before operation was（45.6±26.2）ng/mL,（23.8±12.6）ng/mL,（12.7±7.6）ng/mL,respectively,which was higher than those of the control group[（4.2±3.3）ng/mL,（8.9±7.4）ng/mL,（1.5±1.2）ng/mL]（P〈0.01）.The positive rate of CEA,NSE and CYFRA21-1 of lung cancer patients pre-operation was 49.5%,25.6%,52.3% respectively.After combined detection,the positive rate rose to 80.7%.After operation,the serum levels of CEA,NSE and CYFRA21-1[（28.2±21.3）ng/mL,（17.5±14.3）ng/mL,（8.3±5.7）ng/mL] were statistically lower than those before（P〈0.01）.However,these were higher than the control group（P〈0.01）.Conclusion The sensitivity of serum CEA,NSE and CYFRA21-1 in diagnosis of lung cancer was remarkably raised by combined detection,and the three tumor markers were sensitive for post operation follow-up,which were decreased one month after operation.
