磷酸钠盐与硫酸镁对结直肠癌术前肠道准备后肿瘤坏死因子α的影响

摘　　要：目的采用磷酸钠口服液和硫酸镁进行肠道准备后,观察血液中的肿瘤坏死因子α(tumor necrosisfactor-α,TNF-α)的变化。了解2种肠道准备法对肠道黏膜屏障功能的影响,为临床应用提供理论基础。方法将60例结直肠癌患者随机分为试验组和对照组,每组30例,试验组采用磷酸钠口服液进行肠道准备,对照组采用硫酸镁进行肠道准备,2组患者在肠道准备前和手术当日晨测体质量,留取血样各3mL(静脉血),并对血样进行TNF-α测定。结果试验组经肠道准备后血液中TNF-α与准备前变化不大,对照组经肠道准备后血液中的TNF-α明显高于准备前,肠道准备后对照组的TNF-α明显高于试验组(P<0.05)。结论硫酸镁肠道准备法对肠道黏膜屏障的损伤作用明显大于磷酸钠肠道准备法,而磷酸钠口服液对肠道屏障功能影响不大。Objective To observe the changes of the tumor necrosis factor-α （TNF-α） in the blood after bowel preparation with sodium phosphate oral solution and magnesium sulfate, to understand the impact of two bowel preparation methods on intestinal mucosal barrier function and to provide a theoretical basis for clinical application. Methods Sixty patients with colorectal cancer were randomly divided into two groups, namely ： the experimental group （ n = 30 ） and control group （ n = 30 ）. The experimental group used sodium phosphate oral solution for bowel preparation. The control group were treated with magnesium sulfate for bowel preparation. Venous blood samples （3mL） were collected before bowel preparation and the day of surgery, and TNF-α of the blood samples were determined. Results The blood TNF-α had little change after the bowel preparation in the experimental group, in the control group,TNF-α in the blood after bowel preparation was significantly higher than before preparation, and TNF-α in the control group was significantly higher than that in the experimental group after the bowel preparation （P 〈 0.05 ）. Conclusion The intestinal mueosa barrier injury caused by the magnesium sulfate bowel preparation method was significantly greater than that with the sodium phosphate oral sodium phosphate, and sodium phosphate oral solution has little effect on intestinal barrier function.
