摘 要:目的探讨心房颤动(AF)患者幽门螺杆菌(Hp)感染与同型半胱氨酸(Hcy)、血脂水平变化的相关性。方法选择2011年6~12月于台州市第一人民医院收治的心房颤动患者53例为研究对象,另选择55例健康体检者为对照组。分别采用酶联免疫吸附试验(ELISA)以及免疫比浊法检测Hp特异性抗体(Hp-IgG)和Hcy表达水平,并检测血脂,分析Hp感染与血脂、血清Hcy表达变化之间的关系。结果心房颤动组Hp-IgG阳性率(37.73%)与对照组(30.9%)相比差异无统计学意义(P>0.05);血清总胆固醇(TC)(5.48±0.79)mmol/L、三酰甘油(TG)(2.01±0.51)mmol/L、低密度脂蛋白胆固醇(LDL-C)(3.46±0.71)mmol/L、Hcy(20.84±6.73)μmol/L的表达水平均明显高于对照组(P<0.05),而AP组HDL-C(0.96±0.21)mmol/L表达水平明显低于对照组(1.21±0.19)mmol/L(P<0.05)。Hp感染阳性AP患者的血清TC(5.92±0.83)mmol/L、TG(2.31±0.52)mmol/L、LDL-C(3.97±0.68)mmol/L、Hcy(24.65±6.48)μmol/L表达水平均明显高于Hp感染阴性患者(P<0.05),而HDL-C(0.72±0.17)mmol/L表达水平显著低于Hp感染阴性患者(1.12±0.24)mmol/L(P<0.05)。结论 Hp感染与AP本身的发生没有显著相关性,但Hp感染可能通过对血脂代谢和Hcy表达水平的影响而促进AP的发展和演进。Objective To explore the relationship between helicobacter pylori infection and blood-lipoids,homocysteine(Hcy) in patients with atrial fibrillation(AF). Methods 53 patients with atrial fibrillation were selected as AF group and 55 health people as control group from our hospital during June to December 2011. Serum specific antibodies to Hp(Hp-IgG) and Hcy and serum lipids were measured in two groups. The relationship of Hp and the expression of serum Hcy and blood-lipoids were analyzed. Results The positive rate of Hp-IgG (37.73%) in AF group had no statistical significance compared with control group (30.9 % )(P〈0.05 ), and the expression of serum TC(5.48±0.79)mmol/L,TG(2.01±0.51)mmol/L,LDL-C(3. 46±0. 71)mmol/L and Hcy(20. 84±6.73)μmol/L in AF group were higher than those in control group (P〈 0.05). The expression of serum HDL-C(0. 96±0. 21) mmol/L in AF group was lower than that in control group(1.21±0.19)mmol/L(P〈0.05). Those who were Hp- IgG positive had significant higher levels of TC ( 5. 92 ± 0. 83 ) mmol/L, TG ( 2.31 ± 0. 52) mmol/L, LDL-C ( 3. 97 ± 0.68)mmol/L and Hcy(24.65±6.48)gmol/L than those who were Hp-IgG negative(P〈0.05). Those who were Hp-IgG positive had significant lower levels of HDL-C(0.72 ±0.17)mmol/L than that who were Hp-IgG negative (1.12±0.24)mmol/L(P〈0.05). Conclusion There is no significant corelation between Hp infection and AF. Hp infection may be involved in the pathogenesis of AF by increasing the expression of blood-lipoids and Hey.
