慢性牙周炎伴2型糖尿病患者牙周基础治疗后血清白细胞介素一6和瘦素的变化

摘　　要：目的观察慢性牙周炎伴2型糖尿病患者经牙周基础治疗后血清白细胞介素-6（IL-6）和瘦素（1eptin）的变化。方法选择河南省商丘市第一人民医院2009年6月至2012年2月慢性牙周炎伴2型糖尿病患者34例作为观察组，同期慢性牙周炎患者不伴有全身系统性疾病34例作为对照组，两组均进行牙周基础治疗。分别在治疗前和治疗后3个月记录两组患者探诊出血（BOP）、牙周探诊深度（PD）、附着丧失（AL）以及血清IL-6、leptin水平的变化。结果治疗前观察组血清IL-6和leptin水平分别为（3．42±0．31）ug/L及（1．24±1．13）ug/L，均高于对照组[（3．12±0．34）、（1．01±0．50）ug/L]，差异有统计学意义（P〈0．05）。治疗后3个月，两组患者BOP阳性率、PD、AL、血清IL．6和leptin水平均比治疗前明显下降，差异有统计学意义（P〈0．05）。结论慢性牙周炎伴2型糖尿病患者经牙周基础治疗后牙周状况明显改善，血清IL-6、leptin水平明显降低。Objective To explore the effect of periodontal foundation treatment on serum interleu- kin （IL） -6 and leptin levels in patients with chronic periodontitis and type 2 diabetes mellitus. Methods Thirty-four patients with chronic periodontitis and type 2 diabetes mellitus admitted to our hospital from June 2010 to Feb. 2012 served as observation group, and 34 patients with chronic periodontitis alone as control group. All patients were given periodontal foundation treatment. Parameters including bleeding on probing （BOP）, probing depth （PD）, attachment loss （AL）, and serum levels of IL-6 and leptin were measured before and after treatment, and compared between the two groups. Results Before treatment, the serum levels of IL-6 and leptin in observation group were （3.42 ±0. 31 ） and （ 1.24 ± 1.13） p,g/L, which were significantly higher than those in control group （ P 〈 0. 05 ）. After treatment for three months, BOP, PD, AL and serum levels of IL-6 and leptin were significantly lower than those before the treatment in two groups [ （ 3. 12± 0. 34）, （ 1.01 ±0. 50 ） ug/L ] （ P 〈 0. 05 ）. Conclusion The periodontal foun- dation treatment can effectively improve periodontal status and decrease the serum levels of IL-6 and leptin in patients with chronic periodontitis and type 2 diabetes mellitus.
