摘  要:目的观察脑卒中偏瘫患者正中神经、尺神经的超声变化。方法运用超声技术分别检测100例脑卒中偏瘫患者双侧正中神经、尺神经的横截面积(CSA)以及边界。在偏瘫侧与非偏瘫侧、患者双侧与正常对照组之间进行比较,并分析CSA与年龄、病程之间的相关性。结果尺神经患侧较健侧在肘管入口处明显增粗(P<0.05);正中神经、尺神经双侧均较正常对照组明显增粗(P<0.05);患侧与健侧异常检出率差异无统计学意义;CSA与年龄呈正相关。结论脑卒中偏瘫患者周围神经可出现双侧病变,运用超声可观察到CSA的增粗,可用于指导临床诊断和治疗。Objective To investigate the uhrasonographic changes of median nerve (MN) and ulnar nerve (UN) in stroke patients with hemiplegia. Methods 100 cases of stroke patients with hemiplegia were selected, and the CSAs and boundary at multiple sites along the entire length of MN and UN were obtained by uhrasonography. Com- pared the paretic limbs with the nonparetic limbs, and compared the bilateral limbs with healthy people separately, the relationship of CSAs with age and duration time were analyzed separately. Results The CSA of UN was found larger at the entrance of cubital tunnel in the paretic side than that of nonparetic side ( P 〈 0.05 ). Compared with healthy persons, the CSAs of bilateral MN and UN were significantly increased in stroke patients ( P 〈 0.05 ). The CSAs were positively correlated with age. Conclusion Stroke patients with hemiplegia may have bilateral peripheral nerve injury. The uhrasonography of peripheral nerves reveals the enlargement of CSA and can be used to guide clinical diagnosis and treatment.
