摘 要:目的探讨配对盒基因9(Pax-9)在肺腺癌患者血清中的表达及临床意义。方法采用酶联免疫吸附法(ELISA)测定30例肺腺癌患者血清中Pax-9的浓度,同时检测48例良性肺病变(慢性阻塞性肺疾病、肺炎)患者和32例健康者血清中Pax-9的浓度以作为对照。并分析其表达与患者临床病理特征及患者预后的关系。结果肺腺癌患者血清中Pax-9的浓度([147.9±91.7)ng/mL]明显高于肺良性病变患者血清中Pax-9的浓度([95.2±42.4)ng/mL]和健康对照组血清中的浓度([103.6±56.4)ng/mL],差异有统计学意义(P<0.05)。Pax-9的表达水平与患者性别、年龄、吸烟史及淋巴转移等临床病理特征无关(P>0.05),与肿瘤TNM分期显著相关,血清中Pax-9的浓度随TNM分期而升高,Ⅲ~Ⅳ期([173.0±94.7)ng/mL]明显高于Ⅰ~Ⅱ期([97.6±63.2)ng/mL](P<0.05)。Kaplan-Meier分析显示,Pax-9高表达组患者的中位生存时间为22.5个月,低于低表达组患者的中位生存时间24.0个月,差异有统计学意义(P<0.05)。COX回归分析显示,TMN分期和Pax-9表达对肺腺癌患者预后具有显著意义(P<0.05)。结论 Pax-9在肺癌的发生发展中起一定作用,检测其血清含量对肺癌的诊断具有一定的临床意义,其过高表达有可能作为腺癌预后不良的评估指标。Objective To explore the clinical significance of detection of serum levels of Pax-9 in patients with lung ade- nocarcinoma. Methods Enzyme-linked immuno-sorbent assay (ELISA) was used to detect the protein concentration of se- rum Pax-9 in 30 lung adenocarcinoma patients (experiment group).The serum samples of 32 healthy cases and 48 lung benign disease cases were selected as control groups. The relationship between Pax-9 expression and clinicopathologic characteristics including prognosis of patients was analyzed. Results Compare with lung benign and healthy controls, the serum level of Pax-9 in patients with lung adenoearcinoma was significantly high (P〈0.05). The expression of Pax-9 is unrelated to clinical data such as patients ' genders, ages, smoking history and lymph nodes metastasis (/:'〉0.05) , but it is closely correlated to tu- mor TNM stages. The serum level of Pax-9 in patients with stage III and IV lung cancer was significantly higher than that of stage I and II ( P 〈 0. 05). Moreover, univariate prognostic factor analyzed by Kaplan-Meier method indicated that the median survival time for patients with the high serum level of Pax-9 was shorter than that for patients with low level(P〈0.05). The se- rum level of Pax-9 and the TMN stage of tumor signally affected prognosis in COX regression model (P〈0.05). Conclusion Pax-9 is involved in the development of lung cancer. The detection of the serum levels of Pax-9 plays an important role in the lung adenoearcinoma patients' diagnoses. Its too high expression may be a poor prognostic index in lung adenocareinoma.
