大肠息肉824例发病情况及内镜特点分析

摘　　要：目的:分析824例大肠息肉患者的临床资料及结肠息肉的内镜下特点,探寻其中的相关性。方法:收集我院近6年门诊及住院行结肠镜检查的患者,对其中824例结肠镜诊断并有病理证实为大肠息肉患者的年龄、临床表现及内镜下结肠息肉发生部位及大小进行分析总结。结果:检查患者4630例,检出大肠息肉824例,检出率18%,其中男494例,女330例。好发年龄45～65岁,占65.2%。息肉部位分别为直肠28.73%,乙状结肠25.09%,降结肠10.45%,横结肠17.91%,升结肠10.73%,回盲部7.09%。主要临床表现为腹痛腹胀发生率为36.8%;便血发生率为22.6%;大便习惯或性状改变发生率为17.3%。结论:大肠息肉患者多发生在中老年(年龄在45～65岁之间),临床表现以腹痛腹胀、便血和大便习惯或性状改变为主;分布部位以直肠、乙状结肠和横结肠为主。Objectives To investigate the clinical data and the enteroscopic features of 824 patients with colorectal polypus and to analyze their association. Methods The clinical data of all the outpatients and inpatients undergoing colonoseopic examinations in the recent 6 years were collected and analyzed, a total of 824 eligible patients who were diagnosed as colorectal polypus by euteroscopy and were identified by pathology were included in this study. The age, sex, clinical manifestation, and the characteristics of the polyp （location, size, number etc） of the 824 patients were analyzed. Results Among 4 630 subjects who underwent colonoscopy, polypus was found in 824 patients （18%）, including 494 males and 330 females. The age of 65.2% of the patients was ranged from 45 to 60 years. The distribution of the polypus was located in the rectum （28.73%）, sigmoid （25.09%）, descending colon （10.45%）, transverse colon （17.91%）, ascending colon （10.73%）, cecum （7.09%）. Abdominal pain and bloating, hematoehezia, and change of bowel habits were found in 36.8%, 22.6%, and 17.3% of patients, respectively. Conclusions Colorectal polypus occurred more often in middle aged and elderly people （45-65 years）; abdominal pain and bloating, hematochezia, and change of bowel habits were the main manifestations; the lesions revealed by colonoscopy were mainly located in rectum, sigmoid colon and transverse colon.
