乳腺导管内乳头状瘤65例临床分析

摘　　要：目的探讨乳腺导管内乳头状瘤的发病情况及诊治措施。方法对2000年11月至2011年9月经手术后病理证实的乳腺导管内乳头状瘤及乳头状瘤病的病例进行回顾性分析。结果本组65例患者,其中49例患者首发症状为乳头溢液,占75.4%,16例以乳头乳晕区肿块来诊,占24.6%。术后石蜡病理诊断为乳管内乳头状瘤者56例,乳管内乳头状瘤病者5例,乳头状瘤病伴癌者4例,乳腺导管内乳头状瘤的恶变率是7.1%。49例乳头溢液患者行乳管镜检查,检出率为85.7%。结论乳导管内乳头状瘤最常见症状为乳头溢液。术前行乳管镜检查有较高的确诊率。乳腺导管内乳头状瘤病有一定的恶变率。Objective To investigate the characteristics, diagnosis and treatment of the intraductal papillomatosis. Methods The clinical data of 65 patients who had been pathologically diagnosed as having intraductal papilloma or intraductal papillomatesis from November 2000 to September 2011 were analyzed retrospectively. Results Among the 65 patients, 49 （75.4%） had nipple discharge, including light bloody discharge in 32 patients （65.3%） and yellow discharge in 17 （34.7%）. Sixteen patients （24.6%） were diagnosed with masses of the nipple and areola area. Intraductal papilloma was found pathologically in 56 patients, intraductal papillomatosis in 5, and papillomatosis with malignant transformation in 4. Forty-nine patients with nipple discharge were detected by breast Duct endoscopy, with a detection rate was 85.7%. Conclusions Nipple dischange is the most common symptom in patients with intraductal papilloma. Breast Duct endoscopy is a valuable method for the diagnosis of intraductal papillomas. Carcinoma change may be possible in intraductal papillomatosis.
