脊柱软骨肉瘤影像学表现

摘　　要：目的总结脊柱软骨肉瘤患者X线、CT、磁共振成像(MRI)表现,评价不同影像学检查方法诊断脊柱软骨肉瘤的价值。方法回顾性分析济南军区总医院收治的经手术病理证实的5例脊柱原发性软骨肉瘤患者X线、CT、MRI表现,对肿瘤发生部位、侵犯范围、形态、钙化或骨化、强化表现及肿瘤性质进行分析评价。结果 5例患者,4例病变位于胸椎,1例位于骶椎。X线均表现为软组织肿块,其中位于胸椎的4例椎体形态密度改变。CT均表现为椎体骨质破坏并钙化,其中4例合并软组织钙化。MRI表现为椎体破坏并周围长T1、长T2不均匀信号5例,增强扫描花边样强化3例,结节状强化2例。累及椎管并压迫硬膜囊1例。结论各种影像学检查方法对显示脊柱软骨肉瘤的成分及侵犯范围具有不同的优势,掌握脊柱软骨肉瘤的特征性影像学表现有助于提高临床及影像学诊断的准确率,对临床治疗及预后评估具有重要意义。Objective To evaluate the value of different imaging methods for diagnosis spinal chondrosarcoma through concluding the imaging inmfifestations of spinal chondrosarcoma by X-ray, CT and magnetic resonance imaging （MRI）. Methods Retrospective study was applied to 5 cases of spine chondrosarcoma which were pathologically proven with X-ray, CT, MRI examination. Location, invasive range, shape, calcification or ossification, and enhancement of the tumor were analyzed and evaluated. Results There were 4 cases in the thoracic vertebrae, 1 case in the sacral vertebrae. X-ray showed 5 cases of soft tissue mass, vertebral morphology density change in 4 cases. CT showed 5 cases of vertebral bone destruction and calcification, combined soft tissue calcification in 4 cases. MRI showed 5 cases of vertebral body destruction and inhomogeneous low signal on T1WI, high signal on T2WI, enhancement scanning lace example strengthen in 3 cases, nodular strengthen in 2 cases. One case was involved spinal canal and compress dural sac. Conclusions Different imaging examinations have different advantages on display the composition and invasive range of spinal chondrosarcoma. Familiar with the imaging characteristic of spinal chondrosarcoma is helpful to improve the acuracy of the diagnostic in clinical and imaging. It is benefit to clinical treatment and oroznosis evaluation too.
